Objective: To evaluate the clinical application value of multi-detection CT angiography (MDCTA) in the preoperative diagnosis and assessment of Internal Carotid Aneurysms.Methods: Collect 62 patients which conformed having Internal Carotid Aneurysms by DSA or surgeries and analyze their preoperative MDCT conventional scan and MDCT angiography data that generated from GE 64 detection Lightspeed VCT. All of these data was sent to advantage workstation (AW4.3) reconstructed with multi-planar reformation (MPR), maximum intensity projection (MIP), volume rendering (VR) and stacking technique of VR. Compliance rate for aneurysm detection with MDCT angiography was calculated by using DSA or surgical as the reference standard. While, analyzing the size, neck, body, location and surroundings of aneurysm compared with DSA or surgical findings and evaluating the values of MDCT angiography in the preoperative assessment of Internal Carotid Aneurysm.Results: 70 aneurysms were conformed in 62 patients by using DSA or surgeries. The aneurysms size was 5.5 mm. Of 70 aneurysms, 15 aneurysms were smaller than 3 mm, 22 aneurysms were 3–5 mm, 26 aneurysms were5–10 mm, and 7 aneurysms were bigger than 10 mm in maximal diameter. A total of 68 intracranial aneurysms were detected by MDCTA in these 62 cases, Compliance rate for aneurysm detection with MDCT angiography was 97.1%,two aneurysms were missed which size were smaller than 3 mm that located on the origin part of the posterior communicating artery. The different of neck measurement between MDCTA and DSA was less than 1 mm. Then, investigating 38 aneurysms that be clipped in traditional neural surgeries. There have 7 supraclinoid carotid aneurysms and 31 Posterior communicating artery aneurysm. Of 7 supraclinoid carotid aneurysms, 6 supraclinoid carotid aneurysms oppress the ipsilateral optic nerve, consistent with the surgical findings.Conclusion: MDCT angiography is an accurate imaging method for the detection and preoperative assessment of Internal Carotid Aneurysms, in most cases, can be applied in clinical as the conventional method. |
