| Objective:To explore the value of3D-CTA with256-slice spiral CT indiagnosis of intracranial aneurysm by comparing with3D-DSA asgolden standard.Methods:We collected the data of both3D-CTA and3D-DSA from the43cases with subarachnoid hemorrhage(SAH) in our hospital fromOctober2010to December20121. To evaluate the sensitivity, specificity, positive predictivevalue, negative predictive value of3D-CTA in comparision with3D-DSA; To compare the difference between3D-CTA and3D-DSAin the detection of aneurysm.2. With the measurement results of3D-DSA as golden standard,to compare the difference in the measurement of the maximaldiameters of neck and body of aneurysm among3D-CTAVR,3D-DSAVR, and3D-DSA.3. To compare the difference in the clarity of the neck ofaneurysm between3D-CTA and3D-DSA.Results:The sensitivity, specificity, positive predictive value, negativepredictive value of3D-CTA was91.6%,85.7%,97.0%,66.7%in comparison with3D-DSA in detection of the patients withaneurysm.The sensitivity of3D-CTA was90.2%in contrast to3D-DSA in detection of number of aneurysm. The sensitivity of3D-CTA was85.7%for those aneurysms less than3mm, and95.0%for those aneurysms more than3mm.There was of no remarkblestatistical significance in the detection of intracranial aneurysmbetween3D-CTA and3D-DSA.The statistical significance was notremarkble in the maximal diameters of neck and body of aneurysmmeasured by3D-CTA and3D-DSA.(P>0.05).There was of statisticalsignificance between3D-CTA and3D-DSAVR in the measurement ofthe maximal diameters of neck and body of aneurysm(P<0.05).Bycomparing3D-DSAVR with3D-DSA in the measurement of themaximal diameters of neck and body of aneurysm, we can find therewas of no statistical significance between them (P<0.05).The statistical significance is remarkble between3D-CTAVRand3D-DSAVR in the clarity of the neck of anurysm and the parentartery.Conclution:1.There was of no statistical significance between3D-CTA and3D-DSA in the detection of AN,so3D-CTA may be used in thepreoperative diagnoses of aneurysm. 2.The data measured by3D-CTVR can be used in themeasurement of the longest diameters of the neck and the body of theaneurysm.But we should remember thar the data measured by3D-DSAVR was larger.3.3D-DSA is still needed if3D-CTAVR can’t display the neck ofthe aneurysm and the parent artery clealy. |
