Study On Self-management Status And Influencing Factors In Patients With Chronic Kidney Disease (1-3 Stage)

AbstractObjectives:1.To develop the self-management scale of the patients with chronic kidney disease(CKD) (1-3stage).2.To investigate the status of self-efficacy,social support and self-management in patients with CKD (1-3 stage).3.To analyze the influencing factors of self-management in patients with CKD (1-3 stage).4.To explore the correlation between self-management and self-efficacy social support.Methods:A conveniental sample of 50 patients with CKD(1-3stage) was collected.The survey last from October in 2009 to May in 2010.Datas were collected by self-made personal characteristics questionnaire,the Chinese version of self-efficacy for managing chronic disease 6-item scale,SSRS,self-designed self-management questionnaire of patients with CKD(1-3stage). The descriptive statistics,T-test,one way analysis of variance,Pearson correlation and multiple stepwise regression analysis were used.Results:1.Reliability and Validity of the scale(1)Reliability of the scaleThe inventory had satisfying reliability.For the overall scale Cronbach'sαcoefficient was 0.871,The test-retest correlation coefficient of the scale was 0.812.(2)Validity of the scaleThe content validity index(CVI)for the overall scale was 0.907.The correlation coefficients between dimensions and overall of the scale were 0.776～0.902. 2. The average score of self-efficacy was 7.79±1.37 points; the total score of social support was 44.28±8.08 points;the total score of self-management was 87.96±9.31points,score indicator was78.54%, treatment management was the highest dimension among the 4 dimensions.3. There was statistically significant relationship between the self-efficacy of patients with CKD(1-3stage) and the time since diagnosis(P<0.05),the social support of patients with CKD(1-3stage) and current state of job,the way of payment,the state of marriage(P<0.05), the self-management of patients with CKD(1-3stage) and the way of payment,education level (P<0.05)4.The self-managementof patients with CKD(1-3stage) was positively related to self-efficacy and social support.Conclusions:1.The Self-Management Scale of patients with CKD(1-3stage) has good reliability and validity.2.Most patients in chronic kidney disease (1-3stage) have high level of self-efficacy. The time since diagnosis is the main influencing factor of self-efficacy.3.The level of social support of patients with CKD (1-3stage) is higher than ordinary.The major influencing factors of social support are current state of job,the way of payment and the state of marriage.4.The level of self-management of patients with CKD (1-3stage) is in medium to senior lever,it's still need to improve.Besides,treatment behaviors management had the highest score,and there is significant difference in self-management behaviors of patients.The major influencing factors of self-management are the way of payment and education level.5.There is closely relationship between self-management and self-efficacy,self-management and social support,respectively. The patients having higher level of self-efficacy and social support can conduct self-management behaviors better.On the other hand,these changes can enhance the patient's self-efficacy.
