Clinical Analysis On The Pertinence Between Constitution Classification Of TCM And The Prevalence Situation In AMD Patients

Objective:Through counting the distribution frequency of different constitution types of patients with AMD, and analyzing distribution of constitution classification, preliminarily discuss connection intensity between constitution classification of TCM and indexes of biological characteristic, To explicit the internal relations of them.Methods:44 cases of AMD treated by Ophthalmic Dept. of Jiangsu Provincical Hospital of Traditional Chinese Medicine from Apr.2009 to Apr.2010 are registered according to the observations table of constitutional classification and determination, which established by Atl-china Society of TCM (ZYYXH/T157-2009)-《Classification and Determination of Constitution in TCM》, divide the cases into nine types:normal quality,quality of deficiency of vital energy,quality of deficiency of Yang,quality of deficiency of Yin,wet quality of phlegm,damp and hot quality,blood stasis quality,qi-stagnation quality and special endowment quality, and record patients' data, included disease history,clinical syndrome,job,educational level,the systemic disease history and ophthalmic examination (Including orrected vision,ocular fundus examined by direct ophthalmoscope,fundus photography,FFA,OCT), then investigate the distribution features of constitution classification, the internal relations between constitution types and indexes of biological characteristic of patients with AMD, and the related factors of AMD.Results:1.The age stage which has the most number of AMD patients is 60-69 age stages. With the age increasing, the number of AMD patients increase gradually; The ratio of sex is 1.44/1, and with the age increasing, male patients are more than female patients, so age has influence on them. The differences in age and sex between dry and wet AMD has no statistical significance.2.The result of vocational survey shows that the patients who engage in near work, for example, text and design, have the greatest proportion, is 65.91%, and the educational level investigation shows that along with the increasing of educational level, the patient population increases obviously, the university school record's patient occupies the total example number 47.73%.3.Among the 44 AMD patients, there are 36 cases have cardio-cerebrovascular disease, account for 81.82%, including hypertension medical history to account for 47.73%, has coronary heart disease to account for 18.18%, has the high blood fats to account for 9.09%, has Arteriosclerosis to account for 4.55%, has the stroke medical history to account for 2.27%; Other diseases account for the proportion to be small. Moreover, the smoker accounts for 25.00%, drinking wine accounts for 20.45%.4.The distribution of AMD patients' constitution: normal quality and quality of deficiency of Yang account for the highest proportion, it's 36.36% respectively; the third one is qi-stagnation quality, is 6.82%, the next are quality of deficiency of Yin,wet quality of phlegm,blood stasis quality and special endowment quality, every proportion is 4.55%, quality of deficiency of vital energy is 2.27%, damp and hot quality is 0. 5.Synthesis for transformation score of pathologic constitution>30, quality of deficiency of Yang account for the highest proportion, it is 38.64%, next is normal quality, accounts for 36.36%, quality of deficiency of vital energy and wet quality of phlegm account for 29.55% respectively, the fifth is blood stasis quality, is 15.91%, qi-stagnation quality and quality of deficiency of Yin account for 13.64% respectively, the lest are damp and hot quality and special endowment quality, accounts for 11.36% respectively.6.the statistic analysis of relevance between constitutional types and retinal hemorrhage,CNV,edema, exudation shows that, it compares nine kind of constitutional types,the empty constitution and the solid constitution, P>0.05, has no statistical significance, so we can not think that between nine kind of constitutional types, the empty constitution and the solid constitution have difference on the proportion of retinal hemorrhage,CNV,edema and exudation.Conclusion:1.The survey results of 44 AMD patients' constitution classification show that The proportion of empty constitution is higher than the normal quality and solid constitution obviously, Comparing with the constitution distribution rule of normal elderly people, they have difference, quality of deficiency of Yang in AMD cases accounts for the proportion to be higher than the normal elderly people obviously, so the constitutional type analysis has the clinical significance in the AMD research. Moreover, According to survey results, we consider that the quality of deficiency of Yang,wet quality of phlegm and AMD have certain relevance, but because the number of sample is too small for this investigation, we still can not draw a conclusion by this, waits for the further research confirmation.2.we can not think that between nine kind of constitutional types, the empty constitution and the solid constitution have difference on the proportion of retinal hemorrhage,CNV,edema and exudation.3.With the age increasing, the number of AMD patients increase gradually, and male patients are more then female patients, so age has influence on them. The differences in age and sex between dry and wet AMD has no statistical significance.4.the patients who engage in near work, and have high educational background have the greatest proportion, but because the sample size is limited, we still unable to determine that short distance with eye whether is a AMD morbidity correlation factor, waits for going a step further multi-central, the big sample research..5.This research supports heart cerebrovascular disease, smokes and drinks wine are the AMD morbidity correlation factors.
