TCM Constitution Classification And Its Sensitivity To Conbercept Drug Therapy In Wet Age-related Macular Degeneration Patients

ObjectiveThrough statistical analysis of the TCM constitution types of wet age-related macular degeneration(w AMD)patients who came to our hospital for treatment,the distribution of TCM constitution categories and susceptible constitution types of w AMD patien ts were preliminarily estimated,and the effects of different TCM constitution types on the vitreous cavity of w AMD patients were discussed.Sensitivity differences in efficacy-related indicators of conbercept drug injection therapy.MethodsFrom July 2020 to November 2021,158 patients who were initially diagnosed with w AMD in the Department of Ophthalmology of Wuhan Integrated Traditional Chinese and Western Medicine Hospital were collected,and the "Traditional Chinese Medicine Constitution Classification Assessment Scale" was filled out by means of questionnaire survey or telephone return visit,and the TCM constitution was classified.and 112 patients(112 eyes)who received intravitreal conbercept(IVC)injection(the affected eye received monthly,at least two consecutive conbercept(IVC)injections during the period)were divided into groups according to the constitution of traditional Chinese medicine and recorded.Best corrected visual acuity(BCVA)and central retinal-thickness(CRT)measured by optical coherence tomography(OCT)within3 days before initial treatment and at the end of 2 months postoperatively.The visual acuity was checked using the international standard decimal visual acuity chart,and the result was converted into Log MAR of minimum resolution angle for calculation.To compare the distribution characteristics of TCM constitution types in w AMD patients,and analyze the relationship between the sensitivity of efficacy-related indicators and TCM constitution types in w AMD patients who received intravitreal conbercept drug injection.Results1.Among the 158 patients included in this experiment,there were 77 males and 81 females,with an average age of(70.05±8.35)years and an average disease duration of(1.41±0.75)years.The patients with partial constitution accounted for 70.89%(112cases),and the average age was(70.05±8.35)years.29.11%(46 cases)of the patients with the quality of life.Among the partial constitutions,the highest proportion was yin deficiency constitution 22.15%(35 cases),followed by yang deficiency constitution 17.09%(27 cases),idiosyncratic constitution 13.29%(21 cases),blood stasis constitution 8.23%(13 cases),5.06%(8 cases)of damp-heat quality,2.53%(4 cases)of qi deficiency,2.53%(4 cases)of phlegm-dampness,and no patients with qi stagnation.2.A total of 112 patients(112 eyes)received intravitreal conbercept injection,including 54 males and 58 females,with an average age of(70.17±8.62)years,an average course of disease(1.42±0.54)years,and a peaceful quality of 33 There were 24 cases of yin deficiency,19 cases of yang deficiency,15 cases of special temperament,13 cases of blood stasis,8 cases of damp-heat,and no patients with qi deficiency and phlegm-dampness.There were no statistically significant differences in gender composition,eye sex composition,age,and course of disease among the groups(p>0.05).3.After receiving intravitreal conbercept(IVC)injections for2 months,the overall patient’s BCVA(log MAR)was 0.67±0.03,which was significantly lower than the pre-treatment value of0.94±0.04(p<0.001),and the visual acuity was improved.The CRT of all patients after treatment was(295.18±54.59)μm,which was significantly lower than that before treatment(416.23±201.91)μm(p<0.001).4.The BCVA(log MAR)in each group after treatment was 0.62±0.37,0.68±0.30,0.80±0.19,0.70±0.09,and 0.62±0.37.0.68±0.18,0.47±0.47,BCVA(log MAR)in each group before treatment were 0.94±0.47,1.12±0.0.27,1.11±0.0.25,0.73±0.14,0.79±0.28,0.56±0.42,among which the There were statistically significant differences in quality,yin-deficiency quality,yang-deficiency quality,idiosyncratic quality,and blood stasis quality within the group(p<0.05);there was no significant difference in other groups(p>0.05).The post-treatment CRT(μm)in each group of peaceful,yin-deficiency,yang-deficiency,peculiar,blood stasis,and damp-heat groups were 278.42±53.03,308.88±37.29,277.68±64.27,336.29±48.38,296.44.±28.99,245.60±9.13,CRT(μm)before treatment in each group were 293.85±76.41,673.63±245.49,277.68±64.27,365.21±74.31,368.22±35.31,283.60±44.13,of which yin deficiency and yang deficiency There was a statistically significant difference in the curative effect of the,idiopathic and blood stasis groups(p < 0.05),and there was no significant difference in the curative effect of the other groups(p > 0.05).5.The changes of BCVA before and after treatment(after treatment-before treatment)in each group of patients in each group were-0.32±0.27,-0.44±0.27,and-0.44±0.27.-0.31±0.31,-0.03±0.47,-0.12±0.12,-0.10±0.10.There were statistically significant differences in the changes of BCVA before and after treatment among all groups(p<0.001).After-before treatment were-15.42±53.42,-364.75±231.89,-173.47±152.52,-28.93±27.44,71.78±64.30,-38.00±44.36.All groups There was a statistically significant difference in the amount of change before and after CRT treatment(p < 0.001).6.Pairwise comparison within the group showed that there were significant differences in the BCVA changes between the peaceful quality group and the special quality group,the yin deficiency quality group and the special quality group(p<0.01);there was no statistical difference in the other groups.(p>0.05).T here were differences in CRT changes between the Yin-deficiency quality group and the peace-healing quality group,the Yin-deficiency quality group and the special-endemic quality group,a d the Yang-deficiency quality group and the peace and quality group(p<0.05),and there was no statistical difference between the other groups(p>0.05).ConclusionThere are many biased constitutions in w AMD patients.Yin-deficiency constitution,yang-deficiency constitution and idiosyncratic constitution may be the predisposing constitutions of wAMD.In addition,there are differences in the sensitivity of patients with different TCM constitutions to conbercept(IVC)injections.However,the changes of BCVA and CRT were not completely synchronized.If only BCVA was used as an indicator for judging the sensitivity of treatment,the sensitivity of Yin-deficiency and peacefulness was higher than that of idiosyncratic quality.If only CRT is used as the index to determine the sensitivity of treatment,the sensitivity of Yin-deficiency quality is higher than that of peaceful quality,phlegm-dampness quality,idiosyncratic quality,and qi-deficiency quality.Due to the small sample size,limited source of patients,and short study time,this study still needs to be further demonstrated by a multi-center,large-sample prospective study.
