Background:About half of primary lung cancer cases were diagnosed in advanced stage. At this stage, the role of conventional western medicine treatment is limited, and most of them are palliative treatment. The high metastasis rate and recurrence rate of patients with non-small cell lung cancer(NSCLC)often lead to treatment failure,so the prevention of recurrence and metastasis is an important link to improve the growth cycle of lung cancer patients. At present, most patients with lung cancer will have toxic and side effects such as nausea, vomiting, fatigue and bone marrow suppression after receiving radiotherapy and chemotherapy. The imbalance of internal environment such as body Qi deficiency, phlegm dampness and blood stasis has not been fundamentally changed,so the recurrence rate and metastasis rate remain high.Studies have shown that traditional Chinese medicine has unique advantages in treating lung cancer, which can reduce clinical symptoms, delay cancer progression, enhance immune function, improve complications and side effects caused by chemotherapy, such as pain, fatigue, gastrointestinal reactions, etc.,improve the internal environment of patients, and gradually restore the disordered internal environment to a balanced state. The method of fortifing the spleen, dissolving phlegm, invigorating blood and calming the mind is a basic treatment method based on the syndrome differentiation of qi deficiency, phlegm dampness and blood stasis in patients with NSCLC.The key point is to regulate the imbalance of internal environment after receiving western medicine treatment, improve the internal environment stability, and then restore the balance of Qi,blood,Yin and Yang to achieve the purpose of treatment.Aim:According to the basic theory of TCM and Professor Wang Yanhui’s clinical diagnosis and treatment experience, this paper discusses the clinical efficacy of the method of fortifing the spleen, dissolving phlegm, invigorating blood and calming the mind in the treatment of primary lung cancer patients with a pattern of spleendeficiency and phlegm-stasis syndrome based on the real world, studies whether the method of fortifing the spleen, dissolving phlegm, invigorating blood and calming the mind combined with western medicine conventional treatment is better than western medicine conventional treatment alone, and evaluates the efficacy of the method in the treatment of lung cancer. Therefore,it can provide scientific basis for clinical treatment of malignant tumor. Methods:Based on real-world study,to collect and sort out the clinical data of 65 patients with spleen-deficiency and phlegm-stasis syndrome of primary lung cancer who treated in the Xiang’ an Hospital of Xiamen University from November 2019 to December 2020.According to whether the patients were treated with the methods of fortifing the spleen, dissolving phlegm, invigorating blood and calming the mind, the patients who only received the western medicine treatment(chemotherapy, radiotherapy, targeted therapy)were divided into the control group. On the basis of the western medicine treatment, the patients were divided into the experimental group who were treated with the methods of fortifing the spleen, dissolving phlegm, invigorating blood and calming the mind, taking Ciji-Hua’ai-Baosheng Ⅱ Formula,in which the basic traditional Chinese medicine were Fuling,Dangshen,Muli,Suaozaoren,Chenpi,Fried Maiya,Beimu and Danshen. The scores of TCM syndrome,KPS,tumor related markers,Carcinoembryonic Antigen(CEA),Squamous Cell Cacinoma(SCC),NeuronSpecific Enolase(NSE),Cytokeratin-l9-fragment(Cyfra21-1),toxic and side effects.White Blood Cell(WHB),Blood Platelet(PLT)and Hemoglobin(HGB)were mainly recorded in Blood test and chooses the main security index of liver and kidney function which include the Alanine Transaminase(ALT),Aspartate aminotransferase(AST),urea and serum creatinine were recorded before and after treatment,SPSS 21.0 software was used for statistical analysis. Results:Before treatment, there were no statistical differences in baseline demographics, laboratory test results and disease-related clinical symptoms between the two groups(P>0.05),indicating comparability between the two groups.KPS score:both groups after treatment were higher than before treatment(P<0.05),there were statistical differences, but the experimental group difference before and after the treatment was 5.45±7.11,the control group before and after treatment difference was 3.44±6.53,shows that after treatment, the patient’s physical condition was improved, and the experimental group improved effect is better than the control group.Total score of TCM syndrome:After treatment between the two groups by Mann-Whitney test, the total score of TCM syndrome in both groups decreased(P<0.01);In addition,Wilcoxon test showed that the experimental group difference was 4.48±2.82 before and after treatment, and 2.25±2.99 before and after treatment in the control group. The result shows that the improvement effect of the total TCM syndrome score in the experimental group was significantly stronger than that in the control group. Each single symptom score:The results of Mann-whitney test showed that after treatment, the P values of cough, sputum, fatigue, weakness, anorexia, irritability and insomnia in the two groups were less than 0.05,which was statistically significant before and after treatment,Wilcoxon test in the experimental group showed that P values of each single disease were less than 0.05 before and after treatment, which was statistically significant before and after treatment, and the treatment effect was obvious.Wilcoxon test showed that P values of cough, sputum, shortness of breath, chest tightness and chest pain were all lower than 0.05 before and after treatment, showing statistical significance and significant therapeutic effect.P values of fatigue, anorexia and irritability and insomnia were greater than 0.05,showing no statistical difference.Toxic and side effects after chemotherapy:The results of Mann-whitney test before and after treatment showed no significant difference in P value, and Wilcoxon test showed no significant difference in P value. The indexes of platelet and hemoglobin in the control group were decreased before and after treatment(P<0.05),and the bone marrow depression was more serious than that in the test group.Changes in tumor markers:The results were not statistically significant. Conclusion:According to the traditional Chinese medicine diagnosis of spleendeficiency and phlegm-stasis,using the method of fortifing the spleen, dissolving phlegm, invigorating blood and calming the mind, combined with Western medicine treatment on the basis of CHB-II-F can improve the clinical symptoms of the patients,in particular,it can obviously improve the patients’sleep, diet, physical strength and so on,improve the quality of life fundamentally, reduce the toxic and side effects after chemotherapy or targeted therapy, the treatment effect is better and has good security.
