Objective: To investigate the relationship between Matrix Metalloproteinase 9(MMP-9) and Type 2 Diabetes Mellitus(T2DM) patients infected with tubercle bacillus(TB).Subjects and methods: 154 T2DM patients without complications were recruited from June to August in 2004. Their serum was collected aseptically, and the expression of MMP-9 in serum was detected with semi-quantitative Zymography and Western-blot. All patients were divided into two groups according to the expression of MMP-9: low expression group and over expression group. We followed up them 5 years by prospective study, then compared the infection rate of TB and incidence of other complications, and calculated the relative risk (RR). Meanwhile, we collected the serum of 76 T2DM patients infected with TB and 89 T2DM patients without complications from January to October in 2008, tested the expression of MMP-9 with the same methods, and compared the expression difference of MMP-9.Results:The results of cohort study showed that six subjects were lost to follow up, two in the low expression group and 4 in the over expression group. There were 11 patients infected with TB in the low expression group in five years and the infection rate was 13.9 %(11/79). There were 23 patients infected with TB in the over expression group in five years and the infection rate of low group in five years was 33.3%(23/69), significantly higher than that of the low expression group (P<0.05). The RR is 2.4. Meanwhile, the incidence of other complication such as cardiac and cerebral vascular diseases, nephrotic complication in the over expression group was higher than that in the low expression group (P<0.05). The expression of MMP-9 was detected in the serum of T2DM patients infected with TB and simple T2DM patients. And the expression level of MMP-9 in T2DM patients infected with TB was significantly higher than that of in simple T2DM (P<0.05).Conclusion: The expression of MMP-9 in T2DM patients was different. T2DM patients with high levels of MMP-9 are prone to be infected tubercle bacillus and suffer Chronic complications.
