| 1.Objective:Type 2 diabetes mellitus is the most common non-infectious chronic disease,which is considered to be a lack of exercise.As a good means to prevent or treat type 2 diabetes,exercise plays an active role in promoting the health and reducing the incidence of diabetes mellitus.At present,exercise intervention of type 2 diabetes has three forms,namely,endurance training,resistance training and endurance-resistance hybrid training,one training method has curative effect,some scholars believe that the centrifugal resistance training is superior to other forms of exercise.In this study,the eccentric resistance training model was used to study the effect of eccentric training on patients with type 2 diabetes mellitus.2.Methods:Twenty-six patients with type 2 diabetes mellitus were randomly assigned to two groups: the control group and the eccentric resistance training group.Eccentric resistance tra ining group were trained for 8 weeks,3 days per week,training programs: 1-2weeks 12 RM,eccentric contraction against 10s;3-4 weeks 12 RM,eccentric contraction against 12s;5-6 weeks 12 RM,eccentric contraction against 15s;7-8weeks 12 RM,eccentric contraction against 17s.The control group and the eccentric resistance training group before training and after the last training in the morning collected cubital vein blood samples,detection of blood(white blood cells,neutrophils,lymphocytes and platelet count),changes of fasting blood glucose,insulin and Hb A1c,triglyceride,total cholesterol,high density lipop rotein cholesterol,low density lipoprotein cholesterol,matrix metalloproteinase-2,matrix metalloproteinase-9,hydroxyproline and concentration of the creatine kinase.3.Results:3.1 Compared with healthy subjects,the specific changes of clinical parameters in patients with type 2 diabetes is:(1)fasting blood glucose,insulin,and Hb A1 c were significantly higher(P<0.001),triglyceride,total cholesterol,high density lipoprotein-cholesterol and insulin resistance index was significantly increased(P<0.05,P<0.001);(2)white blood cell count was obviously increased(P<0.01),neutrophil count and neutrophil count and lymphocyte count ratio was significantly increased(P<0.05);(3)matrix metalloproteinase-2and 9 concentrations were significantly increased(P<0.01,P<0.001).3.2 Eccentric resistance training group and control group of type 2 diabetic subjects related indicators of metabolic within-group changes were respectively: Compared with 10 weeks ago,(1)eccentric resistance training group subjects with type 2 diabetes mellitus fast ing blood glucose and insulin concentrations were significantly decreased(P<0.05),insulin resistance index was a very significant decrease(P<0.01),Hb A1 c values were significa ntly decreased(P<0.001);(2)in type 2 diabetic subjects in the control group fasting blood glucose,insulin,Hb A1 c and insulin resistance index showed no significant change(P>0.05).Eccentric resistance training group and control group of type 2 diabetic subjects related indicators of metabolic inter-group changes were: Compared with the control group subjects with type 2 diabetes mellitus,eccentric resistance training group significantly decreased fasting blood glucose(P=0.07),insulin,Hb A1 c and insulin resistance index showed no significant changes(P>0.05),but the concentration of insulin,Hb A1 c and insulin resistance index decreased by 37%,7% and 39% respectively.3.3 Eccentric resistance training group and control group of subjects with type 2 diabetes mellitus of matrix metalloproteinase-2、9 and the hydroxyproline concentration within-gro up changes were respectively:Compared with 10 weeks ago,(1)eccentric resistance training group subjects with type 2 diabetes mellitus serum matrix metalloproteinase-2 conce ntration was significantly decreased(P<0.05),matrix metalloproteinase-9 concentration was significantly increased(P<0.05),hydroxyproline concentrations were significantly increas ed(P<0.01);(2)in type 2 diabetic subjects in the control group of serum matrix metallo proteinase-2 concentration is significantly lower(P<0.05),matrix metalloproteinase-9 and hydroxyproline the concentration of no significant difference(P>0.05).Eccentric resistance training group and control group of subjects with type 2 diabetes mellitus of matrixm etalloproteinase-2、9 and hydroxyproline concentration inter-group changes: Compared wit h the control group subjects with type 2 diabetes mellitus,matrix metalloproteinase eccentric resistance training group 2 in the serum concentration was significantly decreased(P<0.05),matrix metalloproteinase-9 concentrations were significantly increased(P<0.01),hydroxyproline concentrations were significantly higher(P<0.001).4.Conclusion:4.1 The eccentric resistance training can reduce fasting blood glucose,insulin,Hb A1 c and insulin resistance index,and increase insulin sensitivity in type 2 diabetes mellitus patients.4.2 The eccentric resistance training lowered the concentration of matrix metalloproteinase-2 in the blood and increased the concentration of matrix metalloproteinase-9.4.3 The eccentric resistance training can improve the symptoms of type 2 diabetes mellit us,which is related to the change of matrix metalloproteinase concentration. |
