Objectives:To analysis the attitude of students, parents and teachers to the children affected by HIV/AIDS and to prepare for the education system to eliminate the negative attitude toward the children affected by HIV/AIDS through the investigation on the attitude to the children affected by HIV/AIDS.Methods:The cross-sectional study with the multistage sampling method was conducted to interview the teachers, students and their parents in 12 elementary or middle schools by distributing the self-designed questionnaires including the demographic characters, HIV/AIDS knowledge and the attitude to the children affected by HIV/AIDS; rate and proportion were used for descriptive study, multivariate analysis of variance were used for univariate analysis while canonical correlation analysis and multilevel model were used for the multivariate analysis; the statistical analysis were performed with SAS software version 9.0 and all analyses were two-tailed test with test level atα=0.05.Results:1. The awareness rate of HIV/AIDS knowledge was (78.10%±16.25%), the question with the lowest accurate rate (49.37%) was "having sex just with a partner can reduce the spread of HIV", while the highest inaccurate rate (33.06%) was "mosquito bites will transmit the HIV" in a total of 1355 students; the percent of negative attitude were lowest on the questions which were "would you like having lessons with the children affected by AIDS?"(16.46%), and "will you keep far away with the children affected by AIDS?"(15.06%), respectively; The findings of the multilevel model showed that the attitude of the students in Henan province or female students were more positive and the attitude was more positive with the age growing up, higher knowledge scores and the more positive parents' attitude; The results of the canonical correlation analysis (r=0.526, P<0.001) indicated that the awareness of HIV non-transmission, especially the question "having a meal with PLWHA (People Living with HIV/AIDS) and AIDS patients will diffuse the HIV" while the HIV knowledge was associated strongly with three kinds of negative attitude which were "isolation of the patients", "violation of the right to education of the children affected by HIV/AIDS" and "lackness of respect for the children affected by HIV/AIDS".2. The awareness rate of HIV/AIDS knowledge was (81.12%±11.73%), the question with the lowest accurate rate (65.92%) was "having sex just with a partner can reduce the spread of HIV", while the highest inaccurate rate (23.18%) "Mosquito bites will transmit the HIV" in a total 1074 useful questionnaires for parents; the percents of negative attitude on everyday life were the highest and the three questions were "would other parents like letting their children have lessons with children affected by HIV/AIDS?" (39.29%), "would you like letting your children play games with children affected by HIV/AIDS?"(39.48%) and "would you like letting you children have dinner with children affected by HIV/AIDS together?"(40.04%); The findings of the multilevel model showed that the parents who were teachers or living in Henan province or older or having the higher education degree or higher HIV-related knowledge scores had the more positive attitude; the canonical correlation analysis (r=0.506, P<0.001) indicated that non-transmission and the sexually transmitted diseases knowledge were the main determinants within knowledge, especially the knowledge question "having a meal with PLWHA and AIDS patients will diffuse the HIV", while the mostly affected attitude were not having dinner with children affected by HIV/AIDS and not giving the chance to let the children affected by HIV/AIDS gain the education.3. The accurate rate of all the knowledge questions were high and the awareness rate was (95.21%±5.58%) among the useful 146 questionnaires in teachers, and nearly all the teachers'attitude toward the children affected by HIV/AIDS were positive while the questions "do you think it is necessary to publish the information about the the children affected by HIV/AIDS?"(12.33%) and "would you like your children learn on the same table with the children affected by HlV/AIDS?"(11.64%) had the highest negative attitude scores; Both the findings of the multilevel model and the canonical correlation analysis showed that there were not significant association between the knowledge, demographic characters and the attitude to the children affected by HIV/AIDS.Conclusions:1. The attitude to children affected by AIDS/HIV was different, most of the students had the caring and support attitude to the children affected by HIV/AIDS, and parents may have the prejudice attitude toward the children affected by HIV/AIDS while teachers had the clearly caring and support attitude.2. Students'and parents'attitude was more positive with the non-transmission and the sexually transmitted diseases knowledge scores higher, and parents'attitude was the determinant of the students', while the female, older, Henan school students had the more positive attitude and the older, higher education level, Henan province, education system parents had the more positive attitude.3. We needed to explore other new methods to improve the attitude to children affected by HIV/AIDS for the higher rate of negative attitude was little associated with the HIV related knowledge.
