Research On Two-Way Referral Mechanism Of Primary Health Service Institution Under Policy Of Coordinating Urban-Rural Development

The state promulgated a series of documents and policies which will promote the development of primary health service institutions in an aim to address the irrational allocation of health resources, to alleviate the current medical status of the people and to better treat the health problems relating to the survival plan. As the key to the revitalization of primary health service institutions and rational allocation of health resources, two-way referral system now gets more and more attention. After several years of practice, two-way referral system demonstrated its advantages in solving difficult condition of seeing a doctor, promoting rational use of medical resources. But a series of problems still persisted in two-way referral system. For example: it is easier to refer patients to higher-leveled hospital from primary health service institution than referring patients from primary health service institution to higher-leveled hospital; the standards of two-way referral system are not unified; the personnel in primary level health services is unqualified, etc. How to establish and improve two-way referral system, give full play to the role of primary health service institutions and promote the development of health services have become urgent problems need to address.This paper retrospectively analyzes the two-way referral system as well as the situation of coordinating the development between urban and rural area at home and aboard, meanwhile the paper summarizes the problem, causes and solutions in operating two-way referral system domestically. On this basis, the paper descriptively introduce the two-way referral system in Chongqing from five aspects namely as the launch and awareness of two-way referral system; referral norms; referral recognition, referral willingness and referral evaluation so long as the circumstances of coordinating the development between urban and rural area in Chongqing. This paper also dwells on the effect of the policy of coordinating the development between urban and rural area on two-way referral system in light of the primary health service institution, higher-leveled medical institutions and patients. Finally, based on results of the analysis and survey, a series of effective policies and suggestions were proposed aiming at building a sound two-way referral system in primary health service institution which in line with the characteristics of new health system reform and health development in china. The primary care and health service system in china will be continuously improved by promoting the two-way referral system.
