BackgroundChronic Obstructive Pulmonary Disesse is a kind of considerable chronic disease of respiratory system that seriously disservice people's health. It becomes a significant public dissostic publem because of its high prevalence, high fatality and heavy economic burden. How to reduce the medical costs, improve the medical effect and improve the quality of life is become a important problem of the COPD treatment. Clinical pathways which started in America in 1970. It's a potentially beneficial management strategy for both improving heal care and decreasing costs while also maintaining or improving quality of care. Clinical pathways developed later In CHINA. In 1990s we started to explore the Clinical pathways. Nowadays, it hasn't immature, specially to be short of the Clinical pathways for the treatment of AECOPD. Guangdong provincial hospital of traditional Chinese medicine start to carry out a traditional Chinese of Clinical pathway for COPD. Through a lot of optimization, it prove that it has great helpness for the medical effect and decreasing costs.PurposeOptimizate Clinical pathway for AECOPD of Guangdong provincial hospital of Traditional Chinese Medicine. and evaluate the effect of it. In order to improve the medical management strategy, decrease the medical costs, shorten the length of hospital stay and better use of the medical resoures.Methods The process of optimization Clinical pathway for COPD, it include that discuss key problem of Clinical pathway, study the bibliography, advice from the specialist and discuss correlate Function Examination Room. Finally obtain the optimization Clinical pathway.The study performed retrospective study of Chronic Obstructive Pulmonary Disease patients which were hospitalized in the Guangdong provincial hospital of optimization before (from January,2010 to May 2010) and after (from October 2011 to March 2011).The main outcome measures were mean length of hospital stay;hospitalization costs;improvements in clinical symptom and so on.Result172 patients, half of modle control group, and others of optimization group. There were no significant difference between the two groups on admission.The mean length of hospital stay was reduced from 12.35±2.629 to 11.56±2.565 days. The total hospitalization cost has reduced from 10611.35±3013.25 to 9564.16±3127.65 yuan. The western medicine costs has reduce from 3691.35±1687.16 to 3188.01±1511.72 yuan. The medical treatment has reduced from 2301.76±840.09 to 1808.88±903.34 yuan. The the costs of inspection, and Chinese crude durg do not have significant differences. Contrast the hospitalized and after treatment, both of patient improvement of dyspnea and mark of clinical symptoms has great improve.ConclusionThe research indicate that after optimization of the clinical pathway, The costs of western medicine, Chinese crude durg, inspection and medical treatment all have been obviously reduce. It also improvements in clinical symptom well.
