Objective: Approach the value of Color Doppler flow imaging evaluation for primary biliary cirrhosis clinicaly.Materials and Methods:1 Object:A group for comparison:40 age- and gender- matched normal volunteers(medical history, physical examination, chemical examination including liver functional and immunological tests check normaly),male 4 and Female 36 cases with a mean age of 41.7±13.8 years.According to the pathological changes, the patients of primary biliary cirrhosis are made to two groups,B group(the early group):stageⅠand stageⅡ, ages are from 25 to 59, including male 2 and female 15 cases with a mean age of 37.9±17.2 years. C group(the late group): stageⅢand stageⅣ, ages from 31 to 71, including male 3 and female 29 with a mean age of 50.2±14.8 years.2 Methods: The liver was examined with the GE Logiq 9 and Philips iU22 ,US instruments with 2~5 MHz transducer. The biopsy device was Bard with 16G biopsy cut needle made in USA.The patients lying in the decubitus position was examed firstly by 2-D ultrasound. The echogenicities of the parenchyma and the liver structures were appeared. The liver blood flow was noted on color dopper flow imaging (CDFI).The puncturatic biopsy operation of liver was made under the guidance of ultrasound. All were diagnosed as primary biliary cirrhosis by biopsy of liver. According to the pathological changes, the patients of primary biliary cirrhosis are made to two groups.3 Statistics: The SPSS 13.0 was used to statistical computations. P values < 0.05 were considered significant.Results:1 The distribution of the pathological stage of primary biliary cirrhosis : stageⅠin 5 cases,stageⅡin 12 cases , stageⅢin 18 cases and stageⅣin 14 cases.The early group (stageⅠandⅡ) in 17 cases (34.7%),the late group (stageⅢandⅣ) in 32 cases(65.3%).2 The different grading of PBC on 2-D US : The liver size, liver structure , echogenicity and liver flow were related with severity, and developing with the process. The echogenicity of liver parenchyma was devreased, the liver structure was chaotic.3 Color dopper flow imaging in primary biliary cirrhosis:3.1 Portal vein (PV): With increasing severity,the portal vein diameter widened.There are no significant in the normal group and the early group . (P>0.05). The normal group and the late group,the early group and the late group were significantly different (P<0.05).With the increasing severity,the portal vein velocity descent.In the normal group ,the early group ,the late group were significantly different (P<0.05)3.2 Hepatic artery(HA): Hepatic artery diameter,there are no significant in the normal group and the early group (P>0.05);the normal group and late group, early group and late group were significantly different(P<0.05).With the increasing severity of PBC,the velocity of systolic phase descent hepatic artery flow velocity in the normal group and the early group , there are no significant in the normal group and the early group (P>0.05);the normal group and late group, early group and late group were significantly different(P<0.05). Hepatic arterial resistance index(RI) inherent,there are no significant in the normal group and the early group (P>0.05);the normal group and late group, early group and late group were significantly different(P<0.05).3.3 Hepatic veins(HV): With the increasing severity of PBC, the Hepatic vein velocity descent, group of normal and early was not significant(P>0.05);the normal group and late group, early group and late group were significantly different(P<0.05).Conclusion:1 The images of liver in PBC were noted on real time 2-D ultrasound can detect the changes of the liver hemodynamic.2 CDFI can detect blood flow perfusion of Liver and the changes of the liver hemodynamic. To evaluate the extent of PBC, diagnosis,an assessment of therapeutic effects in early stage, 2-D ultrasound and CDFI showed more significant for PBC.3 Ultrasound-guided liver biopsy plays an important role to make a identify diagnosis and carry onpathological staging.
