| Objective:experimental study and clinical research were applied to evaluate objectively the efficacy and clarify the mechanism of the treatment of the combination of breviscapine injection and acupuncture, in turn, to investigate the better therapeutic method in treatment of cerebral thrombosis.Methods:(一)Experimental Study48 experimental rats were randomly divided into six groups:normal control group, model group, acupuncture group, breviscapine injection group, acupuncture+breviscapine injection group (hereinafter referred to as acupuncture and herb combination group), Shuxuening injection group. Cerebral thrombosis rat model was produced through injecting thrombin into the internal carotid artery (ICA). Changes of brain index, brain water content and calcium (Ca2+) content in brain were observed on cerebral thrombosis rat which are treated by the combination of breviscapine injection and acupuncture.(二)Clinical Study45 cerebral thrombosis recovery patients from the acupuncture and rehabilitation department of guangdong second hospital of traditional Chinese medicine were involved as study subjects for the clinical study. The subjects were randomly divided into three groups:breviscapine injection group, acupuncture group, acupuncture and herb combination group, each group has 15 cases. Each of the three groups was received breviscapine treatment, acupuncture treatment, acupuncture and breviscapine treatment individially, each group was totally treated for four weeks. The motor function and activities of daily living abilities in patients were measured by Fugl-Meyre (FMA) motor function scale and Barthel Index (BI) score.Results:(一)Experimental StudyThe brain index, brain water content and Ca2+ content in brain of each model group were significantly higher than the normal control group (P< 0.001). The brain index, brain water content and Ca2+ content in brain of each treatment group were lower than the model group, and the differences was obviously significant (P<0.05). The brain index, brain water content and Ca2+ content in brain of acupuncture and herb combination group were superior to the breviscapine injection group, the acupuncture group, and the acupuncture and herb combination group.The results show:the treatment of the combination of acupuncture and herb can significantly reduce brain index and brain water content, lower calcium content in brain.(二)Clinical Study1 The comparison of FMA motor function scale and BI scoreThe FMA motor function scale and BI score of the three groups before receiving treatments showed no statistical significance (P>0.05), which implied good comparability. The FMA motor function scale and BI score of the three groups after 4 weeks treatment improved significantly, and has obviously sicinificance.The FMA motor function scale and BI score of the acupuncture and herb combination group were superior to the acupuncture group and breviscapine injection group, which has statistical significance (P<0.05).2 The comparison of therapeutic effectsAmong the 15 cases of the acupuncture and herb combination group,2 were basically cured,9 had obvious effect,3 had effect,1 had no effect, none had deteriorated, and the total effective rate was 93.33%. Among the 15 cases of the acupuncture group,1 were basically cured,6 had obvious effect,5 had effect,3 had no effect, none had deteriorated, and the total effective rate was 80%. Among the 15 cases of the breviscapine injection group,1 were basically cured,3 had obvious effect,6 had effect,4 had no effect, none had deteriorated, and the total effective rate was 66.7%. The total effective rate of the acupuncture and herb combination group are all higher than the breviscapine injection group and the acupuncture group, the analysis of the rank data comparison among the three group showed statistical significance (P <0.05).Conclusion:The FMA motor function scale,BI score and the total effective rate of the therapeutic effects of the breviscapine injection accompanied with acupuncture group were superior to the breviscapine injection group and the acupuncture group. The combination of acupuncture and herb is more effective than pure acupuncture or pure herb on the treatment of cerebral thrombosis. The combination of acupuncture and herb which has remarkably brain protective effects can antagonize calcium overload, alleviate cerebral edema. |
