Objective:To understand the correlation of HBV genotype,1762/1764 and 1896 mutation as well as p53 gene in HCC for further investigation about the pathogenesis and theoretical basis about prevention, cure and early diagnosis of HCC.Methods:HBV genotype and mutation of 80 samples including normal, chronic HBV carrier, chronic Hepatitis B, hepatitis B Induced cirrhosis and HBV-associated HCC were tested by NAAT and sequencing method, sequenced by the auto genotype analyzer and tested by the quantitative method and chromas program. The quantization of HBV DNA was detected by FQ-PCR. Expression of p53 protein in above-mentioned 5 groups liver tissues was detected by immunohistochemistry staining mesthod and results were judged by Semi-quantitative integration.Used ELISA tested the HDVM of the HBV serums antigen antibody reaction and analyzed the result by statistical methods. All the testing data were processed statistically by SPSS13.0 statistical software package.Results:1,Among the 78 chronic HBV infectors, there were 31 cases genotype B with the proportions 39.7%(31/78), while 47 cases C with 60.3%(47/78). There were no genotype A,D,E,F,G and H found in this research. Genotype C is the main HBV genotype in Yunan province. The proportion of Genotype C in group ASC,CHB,LC and HCC was 42.9%(6/14),50.0%. (15/30),64.3%(9/14) and 85.0%(17/20),and the discrepancy had statistical significance. (χ2=8.29, p=0.04). While the proportion of genotype C was overwhelming in group LC and HCC, but the discrepancy of distribution in the two groups had no statistical significance (p=0.161). Combining of the two groups(26/34) was compared with group CHB and ASC(21/44), and the discrepancy had statistical significance(χ2=6.62, p=0.01). The proportion of genotype C in group LC and HCC was higher than in group CHB and ASC.2,There were 57 cases with BCP mutation and 49 PC mutation in 78 chronic HBV infectors. The proportion of BCP mutation with genotype C in HBV infectors was 57.1%(8/14),53.3%(16/30),57.1%(8/14) and 60.0% (12/20),while PC mutation was 50.0%(7/14),43.3%(13/30),57.1% (8/14) and 55.0%(11/20),both of the two mutation were more than HBV infectors with genotype B,and the discrepancy had statistical significance(p <0.05)except group ASC.3,Expression of p53 protein in group LC was stongest with positive rate 100%, next came group HCC (70.0%),CHB (70.0%) and ASC (64.3%) and the discrepancy among groups had statistical significance(p<0.05). Combining of group LC and HCC(82.4%) was compared with group CHB and ASC(68.2%), and the discrepancy of p53 protein expression in the two had statistical significance(p<0.05). Strong positive cases of p53 protein expression in group LC and HCC were more than group CHB and ASC.4,Positive rate of p53 protein expression in chronic HBV infectors with genotype C in group ASC,CHB and HCC was 57.1%(8/14),56.7%(17/30);60.0%(12/20),which was higher than in genotype B, that was 7.1%(1/14),13.3%(4/30) and 10%(2/20).The discrepancy had statistical significance(p<0.05). The proportion of HBV infectors with positive BCP mutation and positive p53 protein expression in the four groups was 66.7%,71.4%,71.4% and 85.7%, while proportion of positive PC mutation was 66.7%,52.4%,64.3% and 57.1%, both were higher than HBV infectors with negative BCP and PC mutation. Conclusion:1,There are genotype B and C, mainly genotype C existing in chronic HBV infesters (including chronic HBV carriers,chronic Hepatitis B,Hepatitis B Induced Cirrhosis and HBV-associated HCC) in Yunnan Province, and there were no other genotypes found. There is a connection between the genotype of chronic HBV infectors and the clinical spectrum of disease, genotype C has connection with the relatively serious liver disease(Hepatitis B Induced Cirrhosis and HBV associated with HCC).2,There were more BCP and PC mutation with genotype C than B in every group.3,Expression of p53 protein in group LC was stongest, next came group HCC,CHB and ASC. Strong positive cases of p53 protein expression in group LC and HCC were more than group CHB and ASC.4,Positive rate of p53 protein expression in chronic HBV infectors with genotype C was higher than genotype B. There was obvious BCP and PC mutation in chronic HBV infectors with positive p53 protein expression. Correlation maybe exists among p53 protein expression, genotype C as well as BCP and PC mutation.5,HBV genotype C, BCP and PC mutation as well as p53 gene are related with the occurrence and development of HCC. But whether there is cooperation among them, whether they are related with disease development, and the definite mechanisms that they advance the ocuurence of HCC are still need further research by extending the numbers of cases and expanding research area.
