Objective:To understand the infection status of the genotype and the testing situation of the virus mutation sites for further discussion about the connection of the genotype,normal mutation sties and HCC of the chronic HBV infectors in Yunnan Province.Methods:Used the NAAT and sequencing method tested the genotype and HBV mutation of 120 samples of chronic HBV infectors including HBV, Hepatitis B Induced Cirrhosis,HBV associated with HCC in Yunnan province.(56 cases of positive HBeAg,64 cases of negative HBeAg,80 cases of male and 40 cases of female) Sequenced by the auto genotype analyzer and used the quantitative method and chromas program to make a conclusion about the testing result Used the(FQ-PCR) tested the quantization of the HBV DNA.Used ELISA tested the HDVM of the HBV serums antigen antibody reaction and analyzed the result by statistical methods.All the testing data were processed statistically by SPSS13.0 statistical software package.The statistical description enumeration data were expressed by NNT constituent ratio,enumeration data two samples constituent ratio tested by fourfold table data x~2 testing. Multi samples constituent ratio tested by x~2 testing of×list lines data. Used the fourfold table precise probabilistic method when the sample size were<40,analyzed data by polychotomous Logistic regression,all the result was monitored by two-sided test.The discrepancy has statistical significance when P<0.05.Results:1,Genotype C was the main HBV genotype in Yunan province, 67.5%(81/120),the proportion of genotype B was 22,5%(39/120),there were no genotype A,D,E,F,G,H,I have been found.The distribution of HBV genotype in CHB,LC and HCC groups were 24(40.0%),8 (20.0%),5(25.0%) cases of genotype B.36(60.0%),32(80.0%),15(75.0%) cases of genotype C,the discrepancy has statistical significance.(P<0.05).While the proportion of genotype C was overwhelming in the liver cirrhosis and HCC infectors,combined the two diseases then compared it with the chronic HBV,the proportion of genotype C in the liver cirrhosis and HCC infectors was obviously higher than in chronic HBV group,the discrepancy has statistical significance.2,The proportion of Genotype B and C were 33.9%(19/56) and 66.1% (37/56) in HBeAg chronic positive HBV infectors.31.3%(20/64) and 68.7%(44/64) in HBeAg chronic negative HBV infectors.The discrepancy of the two genotype groups has no statistical significance. (P>0.05)3,There were 23(28.8%) cases of genotype B and 57((71.3%) cases of genotype C among 80 male infectors,and there were 16(40.0%) cases of genotype B,24(60.0%) cases of genotype C among 40 female infectors.The discrepancy of the different genotypes distribution in male and female infectors has no statistical significance after the statistical analysis.(P>0.05)4,The variation testing result of the BCP,PC and C sites among 120 cases chronic HBV infectors:the genotype B mutation in BCP,PC and C were 13,16,6 cases,the genotype C mutation in the above three sites were 55,43,21cases.The mutation in BCP in genotype C infectors was more than in genotype B infectors.The discrepancy has statistical significance. (P<0.05)The distribution discrepancy of the PC and C site mutation in genotype B and C has no statistical significance.(P>0.05)5,The distribution of BCP mutation in CHB,LC and LCC groups were 4,4,5cases in genotype B,21,28,6 cases in genotype C.The BCP mutation in genotype C was higher than in genotype B,the discrepancy has statistical significance.(P<0.05)The distribution of PC mutation in CHB,HCC and LC groups were 8,2,6,cases in genotype B.20,14,9cases in genotype C.The PC mutation happened in genotype C in CHB,LC and HCC groups were higher than in genotype B.the discrepancy has statistical significance.(P<0.05).The distribution of I97 mutation in CHB,LC and HCC groups were 0,5,1 cases of genotype B and 10,6,5 cases of genotype C.The I97 mutation in genotype C in CHB and LC group were higher than in genotype B,the discrepancy has statistical significance.(P<0.05).But the discrepancy of I97 mutation in genotype B and C in LC group has no statistical significance.(p>0.05)6,The BCP and PC mutation in chronic positive HBeAg infectors were 46.4%and 21.4%in chronic negative HBeAg infectors were 65.6%and 73.4%,the discrepancy has statistical significance.(P<0.05)7,Separated the 120 HBV infectors as two groups,one was 20 cases HCC infectors,the other was 100 cases non-HCC infectors.Regression analyzed the multiple variable factors,and the result showed that genotype C,virus mutation,negative HBeAg all have close connection with the HCC.Conclusion:1,There were genotype B and C,mainly genotype C existing in chronic HBV infesters(including chronic HBV,Hepatitis B Induced Cirrhosis, HBV associated with HCC) in Yunnan Province,and there were no other genotypes have been found in cases of Yunan province.2,There was a connection between the genotype of chronic HBV infesters and the clinical spectrum of disease,genotype C has connection with the relatively serious liver disease(Hepatitis B Induced Cirrhosis and HBV associated with HCC). 3,The distribution of the genotype C between the positive HBeAg and chronic negative HBeAg infectors were similar.4,Constituent ratio of the HBV genotype has no connection with the gender.5,The BCP mutation in genotype C were obviously more than in genotype B.BCP mutation in HCC,LC and CHB groups,genotype C were all more than genotype B,in CHB,LC and HCC groups,the PC mutation in genotype C infectors were obviously more than in genotype B infectors.In CHB and LC groups,I97 mutation in genotype C infectors was obviously more than in genotype B infectors.6,The BCP and PC mutation in HBeAg chronic negative HBV infectors were obviously more than in HBeAg chronic positive HBV infectors.7,There was a close connection between the genotype C,HBV mutation and the HCC occurrence.It is worth the further researching about the specific mechanisms to find out if the Genotype C,HBV mutation had promoted the deterioration of the disease and lead to the HCC.
