| Health is the eternal topic and the greatest wealth for people. Grasping the health condition of residents, exploring its influencing factors and understanding the demands of community health services are especially important for improving the health level of residents and rationally allocating community health resources.Chaoyang District is the center of politic, economic and technology of Changchun City. According to 2004 the survey of the status of chronic diseases in Chaoyang District, chronic diseases were the major hazards for residents health; Researches of infectious diseases in recent years shows that the incidence of traditional infectious diseases were on the rise, new diseases continued to pass into our region. The health status of Chaoyang District remains to be further improved. The community health services carry out comprehensive prevention of chronic disease and control of infectious diseases epidemic which are the integration point of clinic and prevention. Therefore, the community health services of Chaoyang District have a long way to go.Objectives:Investigating the health condition of residents and the needs of community health service in Chaoyang District is in order to further evaluate the health quality of the population, analyze the health-related factors of health and discuss the needs of community health service. It's purpose is to grasp the health status of residents, guide the district to carry out health education and health promotion activities, do the work of disease prevention and control well, and provide decision-making basis for community health services. The result is to improve their health and Quality of life.Methods:6404 people were investigated who were willing to accept the survey.Adopt the questionnaires of "Community Health Services personal basic information sheet". "hypertension", "follow-up service record sheet of type 2 diabetes" and "the people lifestyle questionnaire".All the investigators had a Uniform training. All valid data were input into Epidata3.0 database and used via SPSS16.0 software for statistical analysis. Measurement data were described by mean±standard deviation and numeration data were described by rate and proportion. The comparisons between groups were expressed as chi-square test. Univariate and unconditional Logistic regression model were used to analyze risk factors of chronic diseases.Results:1. The survey showed that male to female ratio was 1.03:1 and the average age of women was elder than men. Han national accounts for a major part of 98.1%;Most of them received high education, about 71.6%were middle or above; The majority were in marriages (66.7%);Most of residents" income were on middle level, the ratio was 91.1% between 1000～2000 yuan per capita monthly income. The behavior and lifestyle of residents were in good condition overall, but the status of the old people is not optimistic:smoking rate was 3.6%, drinking was 2.6%, poor sleep quality accounted for 1.2% and the proportion of lack of physical exercise was 69.7% in Chaoyang District.2. The prevalence rate of chronic diseases was 8.13%, and the proportion of people suffered from more than two kinds of chronic diseases was 2.62%.The highest four prevalence of chronic diseases were hypertension, coronary heart disease, Diabetes and stroke. The incidence of infectious diseases was 0.12%, the top ranks in infectious diseases was hepatitis.3. Health behavior of the residents was in good condition overall. The analysis of influential factors of hypertension showed that a positive correlation between age and hypertension and OR was 5.926. Not drinking was the protective factors of hypertension. Poor sleep is also a risk factor of hypertension and the value OR was 1.454. The multi-factor analysis of Coronary heart disease showed that female, age, drinking and poor quality of sleep was the main factors and OR values were 1.793,14.819.3.401 and 1.761. The specific results of multiple Logistic regression analvsis of diabetes:the risk of diabetes in women was 1.528 times as men. The older was risk factors of diabetes and OR value was 3.758.4. The residents tend to the demands of health services was prevention and treatment of common diseases; The rate of health insurance coverage was 61.7%; Going to hospital was convenient; Medical expenses mainly concentrated between 0 to 100 yuan (68.2%); hypertension and diabetes were followed up for 94.3%,91.2%; the ratio of medication complianc of hypertension and diabetes were 97.5%,92.5%.Conclusions:1. The prevalence of chronic diseases was low in Chaoyang District, hypertension was the most harmful to the health and the focus of the community prevention; the incidence of infectious diseases was low, hepatitis ranked top of the total number of infectious diseases.2. Residents had good conditions of health behavior overall. The risk factors of Hypertension are elderly, drinking and poor quality of sleep. Older and high income is the main factors of coronary heart disease. Older, female, high income and the residents who do not regularly eat fruits are risk factors for diabetes.3. For investigated residents, the concept of disease prevention and health education was backward, only 19.2% people chose health education; Going to hospital was convenient, medical costs were not high and the main method of payment was health insurance.4. Community Health Service did a better follow-up work in Chaoyang District; the medication compliance of hypertension and diabetes was good.
