Objective: Investigating the awareness of health knowledge and behavior,utilization of community health resources and satisfaction with community health institutions of chronic disease patients in Urumqi,and to analyze the factors affecting the management of chronic diseases in community health institutions and utilization of community health services for patients with chronic diseases,so as to further improve the effectiveness of community chronic disease management and improve the quality of life of patients with chronic diseases.Policies and suggestions to improve the health status of patients with chronic diseases,and gradually to "health 2020" goal.Methods: Self-designed questionnaires were randomly selected from three community health institutions in Urumqi and sent out questionnaires.After data collection and collation,SPSS17.0 was used for statistical analysis,descriptive analysis was used for qualitative data,chi-square test was used for difference test,chi-square test was used for health influencing factors and influencing factors of community health service utilization,and single-factor analysis and bi-classification logistic regression analysis were used for health influencing factors and influencing factors of community health service utilization.The satisfaction of patients with chronic diseases to community health institutions was analyzed by factor analysis and multiple linear regression model.Results: 1.Cognition of health-related knowledge: Hypertensive patients have better knowledge of disease diagnosis indicators than diabetes patients.The awareness rate of normal blood pressure is 63.24%,and the awareness rate of diabetes is 50.81%.((49)<0.05);hypertension and diabetes patients have a poorer understanding of BMI body mass index and daily oil and salt intake.Hypertension and diabetes patients have a perception of BMI body mass index of 31.88% and 32.66%,respectively;hypertension and diabetes patients The awareness rate of daily oil and salt intake was less than 50%;the awareness rate of diabetes for reasonable diet was 61.69%,and that of hypertension was 50.00%((49)<0.05).2.In the cognitive aspect of community health services: the two diseases have a good understanding of regular follow-up,residents' health records,and health knowledge lectures,and the awareness rate is over 80%;the health records of patients with hypertension(87.63% vs 81.54%)The rate of awareness of regular follow-up(82.93% vs 75.00%),health file update(48.08% vs 39.92%),physical examination content(61.85% vs 53.63%)was higher than that of diabetic patients((49)<0.05);two chronic patients The health record update and regular follow-up frequency cognition rate were lower,all lower than 50%.3.In the use of community health services: the use of basic medical services in the community: the two-week visits of diabetic patients(33.06%)are higher than the two-week visits of hypertensive patients(11.50%);the annual hospitalization of diabetic patients(52.82%)is high Inpatients with hypertension(31.36%);both patients with chronic diseases and hospitalization tend to go to secondary or tertiary hospitals.The use of basic public health services in the community: for regular follow-up(81.53% vs 60.89%),regular physical examination(65.85% vs 57.26%),health education(73.34% vs 62.90%),the use of hypertension patients is better than diabetes patient((49)<0.05).There was no significant difference in the utilization of health records of two chronically ill patients(75.44% vs 69.76%)((49)>0.05).4.Influencing factors of health status: factors affecting the health of hypertensive patients: 14 indicators: age,occupation,Marital status,education level,utilization of community health services,distance from family to community health institutions,whether they will sleep regularly,whether they are willing to eat regularly,whether they are willing to control diet,frequency of blood pressure monitoring,knowledge of hypertension,whether to understand daily Salt intake,type of medical insurance,family history.13 indicators affecting the health status of diabetic patients: age,occupation,marital status,education level,family-to-community distance,community health service utilization,whether to understand dietary balance,whether to regularly monitor blood sugar,whether they are willing to control diet,medical insurance Type,whether to strengthen exercise,whether to take medication on time/injection of insulin,whether there is a family history.5.In terms of factors affecting the utilization of community health services: the type of health care patients are the type of medical insurance,the distance from family to the community;the diabetes patients are the marital status,the type of medical insurance,and the distance from the family to the community.6.In terms of satisfaction: the overall satisfaction rate of community health services for two chronic patients is 67.43%,and the satisfaction is low;the factors affecting the satisfaction of patients with two chronic diseases are: medical level,service quality,service.Take advantage of convenience.Conclusion: Two patients with chronic diseases have low awareness of health-related knowledge,and their awareness of community health services has improved,but they have not yet achieved the goal of promoting healthy behaviors.In the use of community health services,two patients with chronic diseases tend to The use of basic public health services in the community,while ignoring basic medical services in the community;the factors affecting the health status of two chronically ill patients and the utilization of community health services are slightly different.Community health institutions should strengthen their
