ObjectiveTo detected the change of health level, quality of life and the anxiety and depression status and its impact factors in diabetic retinopathy (DR). To assessed and compared the affect of surgical intervention on health level, quality of life and anxiety and depression status in 54 patients with proliferative diabetic retinopathy before operation and at one month after operation.MethodsHospital Anxiety and Depression Scale (HADS), Diabetes specificity quality of life scale(DSQL) and the SF-36 were be used for surveying the level of anxiety, depression and healthy quality of life. A healthy age-and gender-matched control group (n= 54) was also recruited. They were required to be≥18 years of age, to have proliferative diabetic retinopathy were scheduled to undergo strabismus surgery. The potential influence of visual function on the improvement of the HADS, DSQL and SF-36 at 1 month after surgery was examined in patients with proliferative diabetic retinopathy (n= 54).ResultsA total of 117 patients with diabetic retinopathy were asked to participate:67 patients with proliferative phase with a mean±SD anxiety score of 7.11±3.38 and 50 patients with non-proliferative phase with a mean±SD depression score of 7.59±3.23. The volunteer and DR patients have the different anxiety and depression status and SF-36 score(proliferative/non-proliferative/control):the anxiety score:9.86±3.21/6.37±3.68/4.01±3.08; the depression score:9.94±3.05/6.79±3.94/4.09±3.17; However, in patients with the retinopathy:9.86±3.21, the:9.94±3.05; SF-36 score:427.64±67.32/501.21±76.02/567.89±77.86. In patients with diabetic retinopathy, anxiety, depression score were related to course of disease and the average monthly income, while there were no related to the age, sex and educational level. In patients with diabetic retinopathy: DSQL score:68.81±1.18 and were positive correlated with age, course of disease, anxiety and depression score and were negative correlated with the average monthly income. In patients with proliferative diabetic retinopathy, preoperative anxiety score:9.79±3.16, preoperative depression score:9.87±3.02; preoperative Health score: 436.16±68.77, preoperative the quality of life score:59.97±1.69, but 1 month after surgery, the anxiety score:6.87±3.11, depression score: 7.09±3.26; Health score:510.23±78.54, the quality of life score was 72.66±2.15.ConclusionThe DR patients have higher incidence rate in anxiety and depression than those in normal control while the patients have lower quality of life and health level than those in controls. Course of the disease and the average monthly income were the significant impacts on the anxiety and depression in patients with DR. DSQL score were positive correlated with course of disease, anxiety and depression score and were negative correlated with the average monthly income. The SF-36 score have several effective factor including education level, anxiety and depression, age, gender, disease duration, the average monthly income in patients with diabetic retinopathy. By surgical intervention, the anxiety and depression morbidity/score were significantly reduced, while the health and quality of life was improved. in patients with proliferative diabetic retinopathy.
