Objective To evaluate the operative technique and clinical outcome of total hip arthroplasty (THA) for the treatment of the high developmental dislocation of hipMethods We treated 62 hip joints of 56 patients with the high developmental dislocation of hip with total hip arthroplasty between January 2001 and January 2010 in the Department of Orthopaedics.56 hip joints of 50 patients were followed up,6 hip joints of 6 patients were lost contact. There were 22 females and 28males, the average age was 53.6 (27-73 years). According to the classification of Crowe, there were 21 (25 hips) in type III,30 (32 hips) in typeâ…£.5 (5 hips) ever had an operation of acetabular osteotomy. All the cementless acetabular cups were placed at the original anatomic location. The average Harris hip score was 41.2 points before operation. All of patients were exposed through the posterolateral approach. The threaded cups were put in or near the level of the true acetabulum in all of cases. Full coating stems were used in femoral side. All patients were evaluated with the Modified Harris Hip Score. Radiographic evaluations were made preoperative and during follow-up.Results No infection or nerve injury occurred. The mean follow-up was 37 months (range 2-83 months). Modified Harris Hip Score improved from 41.2 to 90.0 points. There was no acetabular or femoral component revision because of mal-position orloosening of the prostheses in all patients. Radiographically, all the prostheses were in place, no loosening were detected. All bone grafts were integrated. All the hips acquired union of osteotomy and bone ingrowth. None of the patients had radiographic evidence of aseptic loosening of prosthesis. Conclusions Total hip arthroplasty for treatment of the high developmental dislocation of hip can get better results significantly, although it presents greater technical difficulty.
