Study On The Relationship Between The Expression Of P-gp And Survivin In Cervical Squamous Carcinoma And The Chemotherapy Sensitivity

OBJECTIVE To explore the relationship between the expression of p-gp or survivin in cervical squamous carcinoma and th(?)hemotherapy sensitivity.METHODS1.There are 10 patients with chronic cervicitis,12 patients with CIN,and 42 patients with FIGO stageⅠb2 toⅡb treated with NACT.The expression of p-gp and survivin were deteced by PV immunohistochemistry technique.2.Analyse the relationship between the expression of p-gp or survivin before NACT and age of patients,surgical stage,myometrial invasion,pathological grading or lymphatic metastasis.3.Determine the correlation between the expression of p-gp and Survivin before NACT and the chemotherapy sensitivity.4.Analyse the correlation betwwen the expression of p-gp and survivin before NACT.RESULTS1.The expression of p-gp in cervical squamous carcinoma and CIN are higher leves than in chronic cervicitis(P＜0.05).The expression of survivin in cervical squamous carcinoma and CIN are both higher than in chronic cervicitis(P＜0.05).2.The expression of p-gp in cervical squamous carcinoma before NACT was not associated with age of patients,surgical stage,myometrial invasion,pathological grading or lymphatic Metastasis.The expression of survivin in cervical squamous carcinoma before NACT was also not associated with age of patients,surgical stage,myometrial invasion, pathological grading,but associated with lymphatic metastasis.It is higher in the lymphatic metastasis than in no lymphatic metastasis(P＜0.05)3.The expression of p-gp after NACT is 92.1%(35/38),higher than before NACT(45.2%,19/42).After NACT the expression of p-gp increases.The expression of survivin after NACT is 39.5%(15/38),lower than before NACT(61.9%,26/42),).After NACT the expression of survivin decreases.4.After NACT,31 patients have gain curative effect,the total clinical response ratio of NACT is 73.8%(31/42),4.8%of patients(2/42)with complete remission and 29 patients 69%(29/42)with partial remission is recorded,and compared with 26.2%(11/42)of patients with stable disease or progression.5.The effective rate of patient with p-gp positive expression before chemotherapy is 52.6%(10/19),which was significantly lower than that of patient with P-pg negetive expression(91.3%,21/23)(P＜0.05);the expression of p-gp in effective cases 32.3%(10/31),which was significantly lower than that in ineffective disease cases(81.8%,9/11) (P＜0.05);The effective rate of patient with survivin positive expression before chemotherapy is 61.5%(16/26),which is significantly lower than that of patient with survivin negetive expression(93.8%,15/16)(P＜0.05); the expression of survivin in effective cases 51.6%(16/31),which was significantly lower than that in ineffective cases(90.9%,10/11);The effective rate of patient with p-gp and survivin both positive expression before chemotherapy is 55.6%(10/18),which is significantly lower than that of patient with both negetive expression(93.3%,14/15)(P＜0.05).6.There is significant positive correlation between the expression of p-gp and survivin before NACT(r_s=0.614,P＜0.05).ConclusionThe expression of p-gp and survivin has obvious dependablity with the curative effect of NACT.The more expression of p-gp and survivin,the worse NACT can gain curative effect.
