Objective: To explore the multivariate factors effecting the Barthal index(BI) of patients with acute ischemic stroke.Methods: A prospective, randomized controlled trial was carried out, 606 patients that consistent with selection criteria were randomly divided into group A (n= 299, treatment with Chinese medicine combined with basic therapy) and group B (n= 294, treatment with basic therapy). A 21-days different treatment was carried out respectively in the two groups, and activity of daily living was evaluated in all patients using Bathal Index(BI). The BI scores have been observed at 60-days and 90-days after treatment, and cut off value is 75, the scores higher than 75 is effective, and the scores less than 75 is ineffective. The age, gender, heart rate, breathing frequency, history of smoking, drinking, stroke, hypertension, heart disease, diabetes, high cholesterol, and lesion diameter in MRI were considered independent variable effecting the BI at 60-days and 90-days. The Logistic stepwise regression analysis was completed using SPSS13.0.Results: In this study, 537 patients have completed the final treatment, the BI scores of 526 patients at 60 days time-point and 525 patients at 90-days time-point were evaluated. On BI of patients between the group A and B, there was no significant difference (P>0. 05) at the 60 days time-point, while the BI of patients in the group A is better than in the group B significantly at the 90 days time-point (P<0. 05). At 60 days time-point, the factors which affected the results are gender, history of stroke, high cholesterol, lesion diameter, heart rate, and breathing frequency. At the 90 days time-point, the factors which affected the results are the group A or B, lesion diameter in MRI, gender, breathing frequency.Conclusion: Treatment with Chinese medicine combined with basic therapy can improve activities of daily living in patients with acute ischemic stroke after 90-days. Gender, history of stroke, high cholesterol, and the diameter of lesions were the factors which affected the improvement of activities of daily living after 60-days treatment, and the lesion diameter in MRI, gender, breathing frequency were the factors which affected the improvement of activities of daily living after 90-days treatment.
