| Objective:To observe the change of NT-terminal pro-brain natriuretic peptide(NT-proBNP)in children with incomplete Kawasaki disease and study the value in the diagnosis of incomplete Kawasaki disease(IKD).Methods:We detected the acute- and restoration-stage plasma NT-proBNP level in 36 children with typical Kawasaki disease(TKD)and 20 with incomplete Kawasaki disease(IKD) respectively,by using electro chemiluminescent immunoassay.We also detected the plasma NT-proBNP level in 30 children with respiratory tract infection with fever but without involvement of myocardium and in 30 healthy children,and made a mutual comparison.Meanwhile ultrasound cardiogram examination was performed in 36 children with TKD and 20 with IKD by using GEVIVID7-color Doppler's ultrasound diagnostic apparatus to measure coronary diameter,LVEF,LVFS and Ea/Aa.Results:Aute-stage plasma NT-preBNP in TKD group and in IKD group were dramaticaly higher than that in respiratory tract infection group and healthy children group(u=7.3703,R<0.0005).The level of plasma NT-proBNP had no statistical significance difference in the group of TKD and IKD(u=0.0512,P>0.25).The restoration stage plasma NT-proBNP in TKD group and in IKD group is significantly lower than that in acute stage(respectively u= 4.98,P<0.0005 and u=4.085,P<0.0005).There were no statistical significance in the difference of plasma NT-proBNP in respiratory tract infection group and in healthy children group (P >0.05).Plasma NT-proBNP in the group with coronary lesion and that in the group without coronary lesion are no statistical significance in the difference between TKD group and IKD group(respectively u=0.1522,P>0.25 and T=105, P>0.05).The rate of coronary lesion in IKD e group is significantly higher than that in TKD(X2=4.54,P<0.05).The sensitivity of diagnosing KD with The level of plasma NT-proBNP that higher than 300 pg/ml was 80.4%,and the specificity was 90%.Positive rate of plasma NT-proBNP>300 pg/ml is significantly higher than that coronary lesion(X2=26.45,P<0.005).Conclusions:The plasma NT-proBNP in IKD group and TKD group rose significantly in acute stage,while dropped obviously in restoration stage.Its rising in acute stage has certain specificity.It may indicate that the level of plasma NT-proBNP could be used as a reference mark in the early diagnosis of IKD.Our study shows the level of plasma NT-proBNP may play an important role in early diagnosis and prevention of coronary lesion of IKD. |
