| ObjectiveTo evaluate the effect of Nuanxin Capsule on quality of life in Patients with Chronic Heart Failure.Methods1 60 patients with CHF from the inpatient or outpatient department were randomly divided into two groups, each 30:the matched control group which was treated with western basic medincine and the treatment group with Nuanxin Capsule(3#, tid) in addition to western basic medicine. The treatment time was both three months.2 Use laboratory checks, such as the blood-routine, urine-routine, stool -routine and hepatorenal function as safety index to make sure whether the Nuanxin Capsule has any side-effects or toxicity.3 Evaluate the scores of 36-item Short Form Health Survey(SF-36) and The Minnesota Living With Heart Failure Questionnaire(MHFQ) before and after the treatment.4 Assess the grade of CHF according to NYHA-Class before and after the thratment and observe the relativity of NYHA-Class and quality of life(QOL).5 Observe TCM symptom score, total effective ratios and NYHA-class. Before and after the treatment, making a statistical disposal and comparing the curative effects.Results1 The comparisons between the quality of life score within different grades of NYHA-Class each other:1.1 Before curing:between different grades of NYHA-Class, there is significant statistic difference(P<0.05). Making the Pearson related analysis of the NYHA-Class and SF-36 and MLFQ as well, SF-36 score had a significant negative correlation with NYHA-Class: the related coefficinent r is -0.82, (P<0. 01) ;yet MLFQ score had a significant positive correlation with NYHA-Class: the related coefficinent r is 0.869, (P<0.01).1.2 After curing: the result of the correlation between NYHA-Class and SF-36 score or MLFQ score is almost the same:the related coefficinent r is -0.885, (P<0. 01) and 0.96, (P<0. 01).Result hint: the score of QOL at different heart function ratings has obvious difference, and there exists negative relativity of NYHA-Class and SF-36 score, yet positive correlation of NYHA-Class and MLFQ score.2 After cure, quality of life in the matched group had improved compared with that before curing and the difference had statistic significance(P<0. 01). In addition, compared with matched group, the improvement of quality of life in the treatment group had showed better result with statistic difference(P<0.05).Result hint:quality of life had improved in both groups and the treatment group had advantage.3 Before curing, making the Pearson related analysis, TCM symptom score is positively related to MLFQ score with the coefficient r is 0.622, while after curing the coefficient r is 0.670. However, TCM symptom score has negative correlation to SF-36 score with the coefficient r equal to -0. 771 before curing and after curing the coefficient r is -0.689.4 Compared with the matched control group, there is significant statistic difference(P<0. 05) in the improvement of NYHA-Class: hinting the treatment group has advantage in this aspect.5 Safety monitoring: after curing, the result in every laboratory check seems to have no difference with that before curing. As we haven't discovered depravation in the research, the result hint the Nuanxin Capsule has good safety and no obvous or serous side-effects.Conclusion1 Quality of life score is related to NYHA-Class closely. Scores of SF-36 has negative correlation with NYHA-Class; yet scores of MLFQ and NYHA-Class exist positive correlation:hinting quality of life meassrued by questionnaire can be used to evaluate the result of treatment for CHF.2 The TCM symptom score that used to evaluate the TCM treatment for CHF has closely correlation to quality of life score.The result hints to introduce the quality of life to evaluate the treatment result of TCM may be meaningful.3 Nuanxin Capsule has an accurate curative effect on improvement for quality of life in patients with CHF and has advantage in the clinical practice, additionally, without side-effects and toxicity. |
