Cornea Sensitivity, Ocular Surface Changes, Expression And Significance Of Metalloproteinases-3 Matrix, Intercellular Adhesion Molecule-1, Human Leucocyte Antigen-DR In Severe Obstructive Sleep Apnea-hypopnea Syndrome

Objective: To explore pathological changes of ocular surface in severe obstructive sleep apnea-hypopnea syndrome(OSAHS). The prevalence of floppy eyelid syndrome in severe OSAHS were checked. Impression cytology was applied to analyses the pathological change of ocular surface in severe OSAHS,the checking list including the grading of squamous metaplasia,the expression of metalloproteinases-3 matrix,intercellular adhesion molecule-1, human leucocyte antigen-DR. Corneal sensitivity was measured and analysed.METHODS:There were 28 patients in severe OSAHS group, 20 male and 8 female with mean age of 35.50±8.60 years.There were 20 patients in control group ,12 male and 8 female with mean age of 36.78±9.32 years, they were free of ocular surface diseaes.The checking list including:1.The severity of floppy lids. 2.The grading of squamous metaplasia by impression cytology. 3. The expression of metalloproteinases-3 matrix,intercellular adhesion molecule-1, human leucocyte antigen-DR on conjunctiva by immunohistochemistry combined with impression cytology. 4.Corneal sensitivity were measuered with Cochet—Bonnet esthesiometer.Results: 1. The severe OSAHS group showed high prevalence of floppy eyelid syndrome (85.71%). 2. The severe OSAHS group showed higher grading of squamous metaplasia compared to normal control group (P<0.01). 3. The expression of Metalloproteinases-3 matrix, intercellular adhesion molecule-1, human leucocyte antigen-DR in conjuntival epithelial cell in severe OSAHS group was positive, while they were nagative in control group (P<0.01). 4. The mean value of corneal sensitivity in severe OSAHS groups was 35.96±7.08mm which was lower than that of 55.76±3.66mm in normal control group (t=12.68,P< 0.01).Conclusion: Severe OSAHS showed high prevalence of floppy eyelid. Compared to normal control group, severe OSAHS had higher grading of squamous metaplasia,more positive in the expression of metalloproteinases-3 matrix, intercellular adhesion molecule-1, human leucocyte antigen-DR in conjuntival epithelial cell and low corneal sensitivity.
