| Diabetic peripheral neuropathy(DPN)is one of the most important and most ordinary complications in type one diabetic mellitus and type two.Its disease incidence rate has the symptom to be possible to reach 50～70%,which occupies the majority of diabetic neuropathy,even more higher.Diabetic peripheral neuropathy is one of the most important factors to decide the prognosis of diabetes mellitus.If a majority of DPN patients have not acquired treatment in time,something with skin ulcer,necrosis and even disable person will developed finally.But Traditional Chinese Medicine on the differentiation and treatment has displayed pleased tendency.Depending on cleaning up a great real of literature and clinical report,integrating the clinical experiences of my tutor,we use Modified Danggui sini decoction on Diabetic Mellitus.This study may offer crucial thinking route and methods for the TCM treatment on DPN for the future.So the meaning of this study beared the importance and clinical guiding value.Objective:By the way of clinical observation,the objective of this study was to prove the validity and security of the Modified Danggui sini decoction on Diabetic peripheral neuropathy,to discussing the process mechanism,to find the priority in comparison with the western medicine.Methods:Prospective randomized controlled experiment,60 Diabetic peripheral neuropathy patients up to the diagnosing standard were divided into the experiment group and the control group,both of which received basic diabetic treatments.The experiment group were given the TCM self-prescribed Modified Danggui sini decoction,while the control group were given western medicine called Methycobal pellets.The treatment and observation lasted 4 weeks.After the 4 weeks,the clinical conditions before and after treatment were evaluated, and the major laboratorial indexes were statistically analyzed,such as level of the FBG and PBG,muscle electrical graph,and so on.Results:(1)Clinical Curative Effect:The clinical comprehensive curative effect between the two groups had statistically significant difference.The experiment group was better than the control group(P＜0.01).Modified Danggui sini decoction was prominently better than the control group in which improvd the DPN patient in cold and pain on joints,the extremity numb,the limbs ache,the shake feeling of thumb,the reflection of ankles and so on(P＜0.01). In the clinical curative effect aspect,marked effect of experiment group were 14 examples(to account for 46.7%),the effective were 12 examples(to account for 40%),the invalid were 4 examples(to account for 13.3%),the total effectiveness was 86.7%;Marked effect of control group were 8 examples(to account for 26.66%),the effective were 11 examples(to account for 36.67%), the invalid were 11 examples(to account for 36.67%),total effectiveness 63.33%.(2)Major Hematological Indexes:Treats the latter two groups of FBG and PBG were obviously to drop(P＜0.01),two groups did not have prominent difference(P＞0.05);total cholesterol(TC),triglycerides(TG),low density lipoprotein in-cholesterol(LDL-C)level of experiment group reduced(P＜0.01, P＜0.05),high density lipoprotein in-cholesterol(HDL-C)did not obviously changes(P＞0.05),control group was not function of improving blood fats(P＞0.05).(3)Neural conductive velocity:After the treatment the sense nerve conduction velocity(SNCV)and the motor nerve conduction velocity(MNCV)had improved(P＜0.01),the experiment group was better than the control group (P＜0.01).Conclusion:Modified Danggui sini decoction was able remarkably to the improvement blood fats,enhancing the neural conductive velocity and the fasting C peptide level.The clinical curative effect of Modified Danggui sini decoction decoction on DPN was prominently better than that of the control group of Methycobal.This function was possibly concerned about improving the DPN patient blood Theology,improving blood fat metabolism,increasing nerve blood current capacity. |
