Influence Of Non-invasive Positive Pressure Ventilation On Respiratory Drive And Muscle Strength In Chronic Obstructive Pulmonary Disease

objective:To explore the influence on each index of pulmonary function,respiratory central drive(P0.1),respiratory muscle strength(PImax,PEmax) and blood gas analysis in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) TypeⅡrespiratory failure through early intervention and treatment of non-invasive positive pressure ventilation(NIPPV).Subject and Materials:56 patients with AECOPD TypeⅡrespiratory failure were randomly divided into common treatment group(30 cases),and intervention group with non-invasive positive pressure ventilation(26 cases).The changes of pulmonary function,respiratory drive,respiratory muscle strength and blood-gas analysis were observed in the two groups before and after the treatment.Results:(1) Both groups had the condition of higher respiratory drive,and lower respiratory muscle strength before treatment,While each index of the two groups showed no statistical difference.(2) After treatment,patients in the common treatment group had a decrease in P0.1,P0.1/PImax,PaCO2,an increase in PImax,PEmax, pH,PaO2 and no improvement in FEV1.0(%) and FEV1.0/FVC.(3) The intervention group with NIPPV showed improvement in P0.1,P0.1/PImax,PImax,PEmax,pH, PaCO2 and PaO2,compared to the common treatment group,it also had an increase in FEV10(%),which indicated statistical significance(P<0.01).(4) Compared to each index in two groups before and after the treatment,the group with NIPPV had a better improvement in PaCO2 and PaO2 than the common treatment group after the short-term treatment.(P<0.05),but had no difference in P0.1,P0.1/PImax,PImax, PEmax.Conclusion:(1)In both the common treatment group and the group with NIPPV, they had an improvement in P0.1,P0.1/PImax,PImax,PEmax,and blood gas analysis,which showed statistical significance.(2) The group with non-invasive positive pressure ventilationcan had a better improvement in PaCO2,PaO2 and FEV1.0(%),while it did not have an obvious improvement in respiratory central drive and respiratory muscle strength.
