Objective This study can mainly identify two problems: What's the important risk factors which influence type 2 diabetes mellitus complicating depression? approaching the neuroendocrine alteration of type 2 diabetes mellitus complicating depression in ACTH in order to understand the possible biological mechanisms.Methods We collect 75 patients with type 2 diabetes mellitus (including 45 patients with type 2 diabetes mellitus and depression, 30 patients with type 2 diabetes mellitus patients without depression) and 30 health adult ages 45-65 years old. The first part: Approaching the important risk factors which influence type 2 diabetes mellitus complicating depression: According to depression, we divide the 75 patients with type 2 diabetes mellitus into two groups: one group with depression; the other group without depression. We respectively collect the risk factors which probably influence type 2 diabetes mellitus complicating depression, including gender,age,body weight,body height,marriage state,race,BMI,FBG,HbA1c,through case control study, then undertake statistical analysis to determine the risk factors influencing type 2 diabetes mellitus complicating depression in these factors. The second part: Approaching the neuroendocrine alteration of type 2 diabetes mellitus complicating depression in ACTH: Dividing all the objects into three groups: one group includes patients with type 2 diabetes mellitus and depression; the other group includes patients with type 2 diabetes mellitus and without depression; the normal control includes 30 health adult. We determine the ACTH value of each object in waking and empty stomach state at 7:00 clock and undertake statistical analysis.Results The level of serum FBG was higher in the group with depression than in the group without depression(p<0.05);There was significant difference between the group with depression and the group without depression in BMX(p<0.05)or HbA1c(p<0.05); There was no significant difference between the group with depression and the group without depression in age(p>0.05)or gender(p>0.05). Logistic regression analysis showed that FBG,BMI and HbA1c were the important risk factors in patients with type 2 diabetes mellitus and depression except age and gender. Student-Newman-Keuls(SNK)analysis showed that the level of serum ACTH was higher in the group with depression than in the group without depression or the normal control (p<0.01),There was significant difference in the level of serum ACTH between the group without depression and the normal control(p<0.01). Conclusion Age and gender aren't the risk factors in patients with type 2 diabetes mellitus and depression; obesity,poorer glycemic control, high level of serum FBG are the important risk factors in patients with type 2 diabetes mellitus and depression. Perhaps neuroendocrine alteration is one of the biological mechanisms in patients with type 2 diabetes mellitus and depression.
