A Case Study Of Health Development Of Wushan County In Public Finance Framwork

This research wants to explain the relationship between finance system reform and health development in rural area. And analyses that in the public finance system how should government do to improve the health development in Western Poverty Areas (WPA), in order to advance the equality of health service.MethodsThis research takes WuShan County in Gansu Province as the behalf of WPA, concerning the main problem currently existed in WuShan's health development, studies the procedure of its finance system reform, analyses its finance income and expenses, as well as their impact in social affairs, and finally presents the county's influence of finance system on native health development. By typical cases study, we try to explain how these problems arise along with finance system reform, and propose a possible means to improve health development in these areas during the public-oriented development of finance system.ResultsIt is found that finance organization in county and township has been mostly centralized during the reform, thus increases its financial dilemma. Nearly all the country sides in WPA are short of finance capability and can not afford local social affairs. No clear distinction has been made to various levels of government when defining their rights and obligation in the health development, so that funds cannot be guaranteed. What is more, any efforts pushing health organization to market has proved only to enlarge the gap between countryside health development in WPA and that of eastern and mid areas. Other factors also contribute to this situation. Defects in financial transfer system make it hard to effectively eliminate the negative influence of finance imbalance, and the absence of an available funds management mechanism reduces the capital utility.ConclusionsConsidering the public-orientation development of China finance system, we propose several suggestions for the problems mentioned above, trying to build better condition for health development in WPA and promote their development. These suggestions are brought up from the finance system point of view and accords with public financial frame, which can be listed as follows.First, Separate the WPA health development obligation of supervision from finance expenditure at least for a period of time.Second, displace the obligation of finance expenditure to superior finance organization.Third, establish an effective transfer payment mechanism for WPA.At last, all the financial support to health development must be managed in an effective way commonly accepted by exchequer and ministry of health.By such means, we believe, the money needed for health development in WPA could be guaranteed, and financial devotion utility could be heightened as well, therefore we can make better condition for health development in WPA under public finance framework.
