Objective: To study the epidemic situation and characteristic of K1subtype and K15 Alleles of Human herpesvirus 8 in Kaposi′s Sarcoma in Xinjiang, and primary discuss correlations between different clinics of Kaposi's sarcoma and different subtypes.Methods: HHV-8 DNA was extracteded with phenol-chloroform-isoamyl alcohol from 27 cases formalin-fixed , paraffin-embedded tissue from the People′s Hospital of Xinjiang Uygur Autonomous Region . The K15 gene of HHV-8 DNA were amplified by nested-PCR, and then were sequenced . Sequentially,the K15 allele was confirmed . At the same time, the K1 gene of HHV-8 DNA were amplified by nested-PCR, and then were sequenced . Deduced amino acid sequences with the software DNASTAR were aligned manually with the program Clustal W . All phylogenetic analysis were carried out by using the PHYLIP package , and then the K1 subtype was confirmed . Correlations between different clinics of Kaposi's sarcoma and different subtypes were determined by Fisher's exact probabilities.Results: (1)HHV-8 DNA was detected in 22 (81.50%) out of 27 cases in Xin Jiang . HHV-8 DNA was detected in 4 cases AIDS-KS (100%) .(2)Phylogenetic studies, based on the partial K1 amino acid sequence analysis, indicate that eighteen of these strains belong to the C subtype , with fifteen belonging to the C2 group and two belonging to the C3 group,but the branch of one case can not been identified. And four strains belong to the A subtype with one belonging to the A1 group , but the branch of three case can not been identified; Nest-PCR-based studies of the K15 genomic region indicate that twenty of these strains were of the P subtype while two belonged to the M subtype . (3) The amino acid sequence of K1(121aa,surrounding VR1)differs by 0~23.31% between HHV-8 isolates. The A and C subtype differ from each other by 7.58% to 23.31% (mean: 14.86%). (4)Four cases AIDS-KS patients were respectively infected by A1,C2 and C3 subtype HHV-8. These strains were all of the P allele . But the two M allele strains were extracteded from classical Kaposi′s Sarcoma patients. (5)The analysis of the biologic and clinical data regarding evolution of KS in our patients show a trend toward some differences between patients with HHV-8 subtype A compared with subtype C . The subtype A displayed significantly more frequent mucosal (75% in A vs 11.11% in C:P<0.05) . (6)The discrepancy of both the K1 subtypes or K15 alleles of HHV-8 among different clinicoepidemiologic forms of Kaposi′s Sarcoma was not significant (P >0.05). Conclusion: (1)The majority of Kaposi′s Sarcoma patients in Xin Jiang were infected by the C subtype and/or P allele of HHV-8 . and C2/P chimera was predominant . But there were some patients infected by A subtype and/or M allele . Remarkably , K1-A subtype and K15 M allele is reported for the first time at home . (2)The subtype A displayed significantly more frequent mucosal than subtype C . Subtype A could thus display more aggressive characteristics of pathogenicity . (3)There were not evident correlations between different HHV-8 subtypes and clinicoepidemiologic forms of Kaposi′s Sarcoma in Xin Jiang .
