Glaucoma is a group of clinical syndromes or eye diseases that threat the vision function of optic nerve and are mainly relative to ocular hypertension. That is to say, intraocular tension exceeds intraocular tissues and especially exceeds the limitation that the retina optic nerve can bear, which will damage the visual function. POAG, a type of glaucoma, is that patho-intraocular hypertension causes the damage to optic papilla and defect of visual field and that angle of anterior chamber is open during intraocular hypertension. Its clinical manifestation is of multiplicity, and the ratio of causing blindness of POAG is very high. So far asⅠknow, as the pathogenesis hasn't been illuminated, there aren't clinically effective treatments. This research is based on referring to domestic and international literature and comprehending the current research situation of glaucoma. Through clinical observation, we objectively evaluate the clinic effect of acupuncture treatment on POAG and wish to provide new means and methods for the treatment of POAG.PartⅠLiterature ResearchThis text has summarized the cognition of name, etiology, pathogenesis, treatment method of POAG in TCM from the period of Huangdineijing to Tang, Song, Yuan, Ming and Qing dynasties and clinical achievements in acupuncture made by modern Chinese traditional physicians. We have also summarized the advanced investigation on epidemiology, etiology, pathology, pathogenesis, diagnosis and treatment. By comprehending the dominance and research directions in POAG at home and abroad, this research aims to provide theory for exploiting external therapy which is safe, convenient, economical, effective and characteristic of Ophthalmology of TCM.PartⅡClinical Research1. Objective: Observe the effect on ocular tension and clinical symptoms in patients with POAG by acupuncturing qiuhou, taiyang, taichong and hegu point, and probe into action methods.2. Method: 25 patients of POAG were included in our research, and 45 eyes were acupunctured. We recorded the ocular pressure before acupuncture treatment, 5, 30 and 90 minutes after treatment and symptoms contrasted with 0.5% Timolol to observe the result of acupuncture treatment.3. Result:(1) After the acupuncture treatment on patients with POAG, the ocular tension was reduced obviously, the comparison between pre-treatment and post-treatment periods showed a statistical significance (P<0.01), while the comparison between acupuncture treatment and 0.5% Timolol eye drop treatment also showed a statistical significance(P<0.01).(2) After the acupuncture treatment on patients with POAG, 6 eyes were completely cured and 37 eyes were cured to certain degree, with a total curative rate of 95.5%.(3) The comparison between the incipient intraocular tension between 41—50mmHg and other group showed a statistical significance(P<0.01). The comparison between the group of patients below 60 years old and another group showed a statistical significance (P<0.01), while the comparison between the group of patients with the disease course between 0.5-3 years and another group also showed a statistical significance (P<0.01).(4) After the acupuncture treatment on patients with the CACG, secondarg glaucoma, ocular hypertension of post-op of anti- glaucoma and normal intraocular tension, there was no statistical significance between pre-treatment and post-treatment.PartⅢConclusion1. Acupuncture is effective on POAG in that it degrades immediately intraocular tension and improves clinical symptom.2. Intraocular tension is reduced in 90 minutes (1. 5h) after acupuncture treatment, the effect of which is better than 0.5% Timolol.3. Incipient intraocular tension, age and disease course influence therapeutic effect of acupuncture. Acupuncture is more effective on the patients less than 60 years old, those with incipient intraocular tension between 41—50mmHg, and those with a disease course of 0.5-3years.4. The effect of acupuncture is not obvious for CACG, secondarg glaucoma and ocular hypertension of post-op of anti- glaucoma, and the influence is little on normal intraocular tension.
