Lung cancer is one of the most common malignant tumors in the world and it threats the health and lives of the people seriously. The high morbility and mortality of lung cancer still have increasing all over the world in recent years. In our country, the mortality of lung cancer is already jumped from the fourth to the first of various malignant tumors and especially in the village. And because our country is abundant in tobacco produce, the number of people who smoke is the most around the world (about 300 million). Especially the number of young people(including students of elementary and high school) who smoke is also increasing steadily, the incidence of lung cancer has a trend of low age, the number of of lung squamous carcinoma and lung adenocarcinoma are increasing obviously. In clinical the early diagnosis of lung cancer mainly depends on imageology and we need the pathology confirm the diagnosis. But sometimes there are some difficulties to obtain a qualified pathologic specimen, and there are also probably certain risk and wound, which is not easy to be accepted by patients and their families. Therefore, a kind of laboratory index with smaller injury, getting material being convenience, examination being fast and having the ability of distinguishing between benign and malignant was very meaningful. Until now, many experiments at home and abroad have already show that the selectin family plays an important role in the metastasis of many kinds of malignant tumors. Thus, in this study we choose soluble P-and E-selectin to discusses the association between them and the metastasis of lung squamous carcinoma and also lung adenocarcinoma, expecting to find a kind of index which can help us to judge benign or malignant, evaluate the state of illness, assess curative effect and prognosis for clinic. Mothod: We collect a total of 38 patients with lung squamous carcinoma and lung adenocarcinoma who visit in respiratory medicine department of China-Japan Union Hospital Affiliated Jilin University during a period from June 2006 to February 2007 as the research objects, and set them as A group. According to TNM classification of lung cancer,we set stageâ… toâ…¢as group A1, stageâ…£as A2,all the patients in group A2 have been found metastasis in different organs. Furthermore, they are divided into lung squamous carcinoma group and lung adenocarcinoma group according to the dissimilarity of pathologic type. At the same time 34 health adults were chosen as comparison group, denoted as C group. We draw blood from them and took blood plasma as specimens then saved according the condition of experiment strictly.The sP-selectin and sE-selectin were detected by enlyme-linked immunosorbent assay. Analyze the relationship among the density of sP-SLT and sE-SLT,TNM classification of lung cancer and the pathologic type,then testify the relativity between SF and the progress of lung cancer.Results:1. The concentration of sP-selectin in blood plasma of NSCLC (including lung squamous carcinoma and lung adenocarcinoma) patients (163.2±35.8ng/ml) is obviously higher than that in healthy people (83.0±28.5 ng/ml), the result has significant difference (P<0.01); The concentration of sE-SLT of NSCLC patients (57.0±7.3ng/ml) is obviously higher than that in healthy people (36.1±13.8ng/ml), the result has significant difference (P<0.01);2. The concentration of sP-selectin in blood plasma of the patients who have other organs metastasis(188.0±15ng/ml) is obviously higher than that of the patients who have no other organs metastasis (146.3±36.1ng/ml) and the result has the significant difference (P<0.01); The concentration of sE-selectin in blood plasma of the patients who have other organs metastasis (62.5±4.47ng/ml) is obviously higher than that of the patients who have no other organs metastasis (53.1±6.41ng/ml) and the result has significant difference (P<0.01);3. Comparing lung adenocarcinoma with lung squamous carcinoma, the sP-SLT in blood plasma of the former (177.8±21.7 ng/ml) is obviously higher than the latter (147.8±41.5ng/ml) and the difference have statistic significance (P<0.05); Comparing the lung adenocarcinoma with lung squamous cell carcinoma, the sP-SLT in blood plasma of the former (60.2±5.9ng/ml) is obviously higher than the latter group(53.9±7.4ng/ml)and have statistic significance (P<0.05).4. In addition, a result should also be pay attention to. In the research objects, among the patients with lung squamous carcinoma, there are 11 people smoke more than 400 cigarettes every year, including 7 males and 4 females, there are 7 persons who do not smoke, including 3 men and 4 women. The smoking group and nonsmoking group does not have obvious difference in age distribution, the stage of illness and gender proportion. After statistical analysis, we can include: Comparing smoking group with nonsmoking group, the sP-SLT in blood plasma of the former is obviously higher than the latter, and the difference have statistic significance(P<0.05),but the concentration of sE-SLT have no correlation with smoking. We may infer the objectionable constituent in tobacco can aggravate the injury of bronchi mucous membrane, promote local infiltration and distant place metastasis. But the number of patients is not enough, so we need advanced study to confirm its confidence. On the whole, we can conclude: The concentration of sE-selectin and sP-SLT in blood plasma of patients with lung squamous carcinoma and lung adenocarcinoma is obviously higher than that in healthy people. It shows that the concentration of the P- and E-seletin have close relationship with the incidence of lung squamous cell carcinoma and lung adenocarcinoma; The concentration of sP-and E-selectin in blood plasma of the patients who have other organs metastasis is obviously higher than that of the patients who have no other organs metastasis, the result suggests that P-and E-selectin have relation with the metastasis of lung cancer; Comparing the concentration of sP-and E-selectin in the patients with lung squamous carcinoma and lung adenocarcinoma, the above two indicators of patients with lung adenocarcinoma are higher and the results have statistic significance. We presume that maybe it has something concerned with adenocarcinoma is abundant with blood vessel, which avail metastasis; The sP-and E-selectin have the advantages that easier to get sample, smaller injury, higher sensitivity, lower costs, etc. If the big sample in-depth research can be made, it is possible to desire the selectins could become an import reference index that diagnose lung squamous carcinoma and lung adenocarcinoma in the earlier period and monitor the states of illness and evaluate the prognosis; The sP-and E-selectin play an important role in the incidence and metastasis of lung cancer, If the advanced research can be made, the treatment purpose that control the lung cancer metastasis by repressing adhesion among the cell of lung cancer, endothelium cell and platelet maybe achieved. |
