Pregnancy-induced hypertension (PIH) is a typical disease at pregnancy period. PIH is a serious complication of pregnancy, which usually happens after the 20th week of pregnancy. It mainly appears as high blood pressure, albuminuria and edema, also it can be accompanied by twitch,twitch (pre-eclampsia and eclampsia),even death of pregnant woman and fetus,and at the same time it may have complications in many other organs. Till now, the cause for PIH hasn't been thoroughly discovered. Research shows that there are several cytokines participated in the PIH and various PIH related cytokines. The super family of EGF and the family of EGFR are now drawn a lot of attention. The EGF family is a group of glucoprotein which has similar structure and biological functions as epidermal growth factor ( EGF). The EGF family consists of EGF,transforming growth factor-α(TGFα),heparin-binding EGF-like growth factor(HB-EGF) ,amphiregulin(AR) ,beta cellulin(BTC),epiregulin,neudifferentiation factors(NDFs) and other growth factors, It has various physical functions, which take very important part in the process procreation of procreation. HB-EGF is one of the new members of this family, which is first found in culture of human macrophage. It contains 86 amino acids,which are positioned on the No.5 chromosome of human. It is a single chain glycoprotein with a molecular weight of 22KD, existing both in dissolvable and un-dissolvable conditions. HB-EGF adopts its name due to the strong affinity with heparin . HB-EGF's biological activity is similar to EGF,however, is activity of accelerating cell division is stronger than EGF, it takes effect through the interrogation between EGFR and HSPG, HB-EGF is a strong cytokine which can accelerate cell division. During the resent years' research, it is found that HB-EGF has a close relationship with incretion of procreation, it has its deposit in endometrium,endometrium and decidua,and stimulates growth and differentiation of endometrium, and implantation,it is good for trophocyte invadation to endometrium,and it's one critical necessity involved in adhere and implantation. HB-EGF also relates to the forming of placenta and its growth and sustaining functions. Low level of HB-EGF will lead to abnormal functioning of trophocyte,affect develop of villus,and cause a bad placenta formation,ischemia and anoxia in placenta,at last it can be developed into PIH. The domestic overseas study of HB-EGF has just started. The studies are more of the effect on the process of embedding. There are not many reports on HB-EGF performance during PIH. There is almost blank no such study in the country. Discussing HB-EGF' performance during PIH will help the further study on PIH so that the theoretical support can be provided to forecast it. This experiment analyzed the different level of HB-EGFperformance in placenta of normal pregnancy and PIH through immunohistology,and different level of HB-EGF performance to prove and further study this factor's effect in PIH. Take the example of 28 pacenta from both normal pregnancy and PIH in the first hospital of JILin university and from July 2004 to January 2005,There are 19 samples of PIH, the other 9 are taken as comparison. The observation is conducted through immunohistolog to evaluate HB-EGF's performance in each group. The result appears positive if there is light yellow to brown yellow color inside cytoplasm. Use half fix quantify grade:①Random select 10 mid eye shot(10×10),estimate the number of eye shot,calculate the total amount. If the number of eye shot is in between 0~25%, count as 0,25%~50% count as 1,50%~75% count as 2,above 75% count as 3. ②observe pigmentation degree under mid eye shot,calculate the total amount . No pigmentation or not so clear counts as 0,light yellow counts as 1,yellow counts as 2,dark yellow counts as 3. ③observe pigmentation condition under different eye shot. No pigmentation under high eye shot (40×10)counts as 0 ,there are positive pigmentation under high eye shot (40×10)counts as 1,there are positive pigmentation under mid eye shot (10×10)counts as 2,there are positive pigmentation under low eye shot (4×10)counts as 3. The final value is the sum of above: 0 records(-),1-3 records (+),4-6 records(++),7-9 records(+++). All result are analyzed by EXCEL,shown as X±S, analyzed by records and ttest. The result indicated:The comparing group and PIH group did not show HB-EGF in fetal side in placenta, both groups showed HB-EGF positive pigmentation in the mother side. Normal pregnancy group's performance is positive (++),HIP group 's performance is weak positive(+).There is a significant difference between both the PIH group and sever PIH and normal(t=2.26,t=2.54,P<0.05 ) . Normal pregnancy group and PIH group's HB-EGF performance both reached mesenchyme cell,Other kinds of cells did not show HB-EGF. Inside HB-EGF positive cells,positive grain all positioned in mesenchyme. HB-EGF is an important pregnancy related protein,not only take part in the implantation,stimulates growth of blastula,but also has pregnancy sustaining functions ,its performance can possibly be controlled by blastula.The HB-EGF performance in placenta of PIH is obviously lower than normal pregnancy,it proves the close relationship between HB-EGF and PIH. HB-EGF and its receptor exist in all kinds of human tissues,in women reproductive system, it can be integrated with EGFR and HSPG,participate in information exchange between cells, stimulate the proliferation and differentiation of cells and generates the transference of cells. HB-EGF takes part in the whole process of pregnancy , adjusts proliferation and differentiation of endometrium, stimulates the growth of placenta and sustain its function.
