Objective: Glaucoma is a kind of disease that seriouslythreat vision, the basic and clinical research ofglaucoma,especially those with regard to glaucoma optic nervedamage and neuroprotection is the keystone in work to fightagainst blindness.This examination is dispart to two part,observeretrobulbar hemodynamic changes and NO level.Disscuss bloodflow,NO changes in AACG patients and its relationship withIOP,then analyze these changes may induce what affect onglaucoma pathological process. ⑴ observe retrobulbarhemodynamic changes in acute angle-closure glaucoma (AACG)patients before and after trabeculectomy, discuss the changes ofretrobulbar hemodynamic' possible influence on glaucomaoccurs and develops. ⑵ measure the nitric oxide level inaqueous humor and serum samples from patients withAACG.Analyze the posibble reason of NO level change andwhat role the NO plays in glaucoma occurs. Methods: ⑴use color Doppler flow imagine measureretrobulbar hemodynamic of 28 AACG patients (31 eye), 20normal person examples (40 eye) . The index measured ofeach blood vessel: Peak-systolic velocity (PSV), end-diastolicvelocity (EDV), and resistance index (RI). AACG beforeoperation and normal group,AACG before and afteroperation,AACG after operation and normal group were studiedwith statistics analysis. ⑵The NO levele in serum of 28 AACGpatients and 20 normal person,in aqueous humor of 22 AACGpatients and 13 age-related cataract patients were detected bynitric acid reductase method. Results: ⑴ Comparison of retrobulbar hemodynamicbetween AACG group and normal group: AACG group vsnormal group, Decreased PSV,EDV and elevated RI wereobserved in CRA and SPCA.The result have significantdifference in statistics(p<0.01). ophthalmic artery representdecreased EDV and elevated RI,but decreased PSV in OA havenot sinificant difference in statistics(p>0.05). ⑵Comparison ofretrobulbar hemodynamic between before and after operation inAACG group:the hemodynamic of AACG group havesignificent improvement after trabeculectomy,PSV,EDV areelevated and RI are decreased.But comparision of SPCA'EDV,RI and OA' PSV,RI have not sinificent differ instatistics(p>0.05). ⑶Comparison of retrobulbar hemodynamicbetween after operation AACG group and normalgroup:Retrobullar hemodynamic(RI and EDV of SPCA) ofAACG group is still inferior to normal group, The result havesignificant difference in statistics (p<0.05). ⑷Comparison of———————————————————————————————————————— 5英 文 摘 要NO level in aqueous humor between AACG group andage-related cataract group:The NO concentration in aueoushumor of AACG group is 61.46±4.89μm/L, The NOconcentration in aueous humor of age-related cataract group is30.60±2.81μm/L, it have significant difference instatistics(p<0.01). ⑸Comparison of NO level in serum betweenAACG group and normal group: The NO concentration inserum of AACG group is 68.49±15.37μm/L, The NOconcentration in serum of normal group is 72.19±10.82μm/L, ithave not significant difference in statistics (p>0.05). Conclusions: ⑴ intraocular pressure can very notablyaffect retrobulbar hemodynamic, AACG patients have moreinsufficiency of blood flow than normal person. EDV of someAACG patients can drop to 0,then ischemia-reperfusion injuryoccurs. ⑵trabeculectomy can effectively improve ophthalmicblood flow,the effect is relevant with IOP drop.CRA have themost obvious change after operation among blood vessels.⑶Though IOP of AACG pati...
