| Objective To investigate the effective methods in treating acute retinal necrosis (ARN) . Methods Treatments of 20 eyes with ARN were analyzed retrospectively, 11 were at acute stage, 9 at late stage.According to the different therapy method, the patients were divided into group A: 7 eyes treated with drugs; group B: 13 eyes underwent laser photocoagulation or vitrectomy and endolaser combined with drugs. According to the duration ,they were divided into acute stage group and late stage group. The patients were followed by for 3 to 74 months. Results Eight eyes became retinal detachment, the incidence of retinal detachment in group B was significantly lower than in group A(p=0. 0044, Fisher probabilities). Visual acuity improved by at least two lines in 8 eyes (40.0%), improvement of visual acuity in group B was greater than in group A(p=0. 0147, Fisher probabilities). Visual acuity of 9 eyes(81.8%) were improved in 11 eyes of acute stage, six of which reached 0.1; visual acuity of two eyes (22.2%) were improved in late stage group, only two of which reached 0.1. Visual acuity of eyes at acute stage was significantly better than that at late stage (p<0. 01, Wilcoxon rank sum test). There was a negative correlation between visual acuity and thickness of neuroretina within 500m of fovea( γ= 0.897,p=0.007,Pearson correlation analysis). Conclusions Early diagnosis, timely treatment of drugs combined with laser photocoagulation and vitrectomy are crucial in improvement of prognosis of ARN. |
