Objective:Pars plana viterctomy(PPV)combined with retinal laser photocoagulation for the treatment of proliferative diabetic retinopathy(PDR)combined with diabetic macular oedema,and intravitreal injection dexamethasone extended-release(Ozurdex)at the end of the suigery.The study evaluated the safety and effectiveness of Ozurdex in the treatment ofPDRMethods:Thirty-fivepatients(41 eyes in total)in the First Affiliated Hospital of Gannan Medical College from December2021 toNovember2022were divided into three groups:intravitreal injection of dexamethasone extended release(group A),intravitreal injection of Conbercept at(group B),and no special drugs(group C).Patients were diagnosed with proliferative diabetic retinopathy complicated with macular edema and they conventional pars plana viterctomy(PPV)with retinal laser photocoagulation.Three groups of patients were observed for 3 months to compare the safety and effectiveness of three different surgical regimens.Results:1.The difference of preoperative best corrected visual acuity(BCVA)among the three groups was not statistically significant(P>0.05),andBCVAafter surgery:groupA>group B>group C(including at 1 day,1 week and 1 month after surgery),the difference was not statistically significant(P>0.05).2.The difference of preoperative central foveal thickness(CFT)among the three groups was not statistically significant(P>0.05),and the CFT of the patients after surgery was counted.CFT of one day after surgery:Goup B<Goup A<Goup C,the differences was not statistically significant(P>0.05);CFT of one week after surgery:Group B<Group C<Group A,the differences was not statistically significant(P>0.05);CFT of 1 month after surgery:Group B<Group A<Group C,the difference was statistically significant(P<0.05);CFT of 3 months after surgery:Group A<Group B<Group C,the difference was not statistically significant(P>0.05).3.There was no statistically significant difference in CFT elevation in group A、B between the day before surgery and after surgery,and a statistically significant difference in CFT elevation in group C.There was a statistically significant difference in CFT reduction in all three groups from 1 day to 1 week postoperatively;there was a statistically significant difference in CFT reduction in group AB and a statistically significant difference in CFT change in group C from 1 week to 1 month postoperatively;there was a statistically significant difference in CFT reduction in group A and a statistically significant difference in CFT elevation in group B from 1 month to 3 months postoperatively and a statistically significant change in group C.4.The rate of cataract exacerbation in group A was higher than the other two groups,the difference was not statistically significant(P>0.05);the rate of intraocular pressurc(IOP)elevation was higher than the other two groups,the difference was statistically significant(P<0.05);the rate of retreatment in group B was higher than the other two groups,the difference was not statistically significant(P>0.05);the rate of postoperative viteous cavity haemorrhage(POVCH)in group C was higher than the other two groups,the differencewas statistically significant(P<0.05).Conclusion:PPV combined with retinal laser photocoagulation is safe and effective in improving visual acuity and reducing macular central recess thickness for patients who have proliferative diabetic retinopathy.Injecting Ozuurdex into vitreous can reduces the risk of POVCH and Ozuurdex can enhance the improvement of macular edema The effect of Ozurdex on intraocular pressure was manageable and the difference in cataract exacerbation was not statistically significant compared to the other two groups.Therefore,it can be concluded that PPV combined with retinal laser photocoagulation and intravitreal iqection of Ozurdex is safe and effective in the treatment of PDR... |
