Analysis On Causes Of Hospital Deaths In Patients With Road Traffic Injury

IntroductionWith the quickly development of cities and motor vehicles, the traffic accident has become " The first social effects of pollution in the world" , which is threatening human life. About 1. 17 million and 1 thousand persons were killed by road traffic injury ( RTI) in 1998. RTI became the tenth cause of death in the world at the same time. The world health organization ( WHO) forecasted that RTI will become the third cause of death in 2020. Since China began to reform and open up, the mobile car increased 27. 37 times in 1998 than that of in 1978, and the motorcycle added to 240. 01 times. RTI becomes the first death reason for citizen and young man now. Traffic medicine has made large progress in China during 20 years. While death reasons of RTI in hospital are complex, and they are impacted by many factors, so the epidemiological characteristics in hospital in accordance with the RTI circumstance incompletely. According to this, we investigate the epidemiological characteristics and relative factors of hospital deaths in patients with road traffic injury during 10 years, in order to define emphases in different clinic phases and reduce mortality rate efficiently.Experiment data and Methods1. Experiment data: We choose RTI cases from Jan, 1992 to Dec, 2001,which come from Fujian Provincial Hospital.2. Experiment standards: According to ICD - 9CM, the death cases attach to E811 ~E816NE819 -E826.3. Experiment items; Including the general states, accidents time,injury methods, Injury score, the death cause of RTI, etc ( totally 21 items).4. Experiment methods of measurement:(1)We use ISS90 to evaluate severity of injury according as AIS90 (2)Then we calculate Ps with TRISS5. Statistical analysis: We studied with Ridit statistics.Result1. There are 239 RTI death cases, including 186 men and 53 women. Their age range from 3 to 80, average 36. 5years, and there are 75 examples belong to the age phase (21 -30 years) , which is max among different age phases. Peasants are 98 ( occupy 42. 78% ).2. The rate of traffic accidents is higher from July to September each year, but it is less during 1 to 3 than others . There are two high point every day, one is 23:00 -1:00, another is 14:00 - 16:00.3. Concussion is the mostly method of traffic accidents, thereinto motor vehicle -foot passenger is major mode, there are 131 examples (occupy57. 12% ). And there are 171 cases that attach to E814 (oc-cupy 57. 64% ) by E classification of ICD -9CM.4. The mostly injury method is impact, which has 171 cases (occupy 74. 67%).5. Parts of injury including: head and neck have 179 cases, limbs have 171 cases, skin has 112 cases, face has 54 cases, abdomen has 22 cases, and chest has 13 cases. 192 examples have wounds overtake two parts (which occupy 83,84% ) , and 71 examples have compound wounds. Injury score: RTS 3. 39 - 15. 34, average 8. 43 ; ISS 11 -59, average 30. 56 .6. Persons who die of RTI are increasing year after year, the proportion occupy the first place among all kinds of trauma7. The relation between time and death reasons of RTI; the death time range from 1 day to 155 days after wounded persons get into hospital, average 5.5 days.. The effect of Ridit statistics show that the time is longer, the mark of R is bigger, which hint the distinct relation between time and death reasons of RTI. The history in hospital of RTI can be divided into 4 phases: First <1 day(118 cases) , Second 2 -7 days(87 cases) , Third 8-14 days(16 cases) ,The last >15 days (18 cases).Discussion1. Epidemiologic feature:1. 1 On the age side, most death persons of RTI are young people or middle age people, which accord with wounded persons of RTI whose age range from 15 to 44 years, and the wounded number of elderly whose age exceed 60 years increase year after year.
