Clinical Study On Chronic Renal Failure And Effect On Plasma Endothelin Treated With Shenmai Dihuang Decoction

Objective: In the study, the TCM theories of treatment according to syndrome differentiation were used to probe into the etiology, pathogenesis, management method and prescription of chronic renal failure(CRF), and to find out the efficient way of treating CRF. Based on years of clinical experiences, my tutor proposed that the main mechanism of CRF in early and medium stage is deficiency Qi and Yin of spleen and kidney, combined with damp, toxin and blood stagnation. So invigorating spleen and tonifying kidney , nourishing Qi and Yin , assisted with dispersing dampness and toxin, and removing stagnation is proposed as the main treatment principles of it. Guided by these principles, we made up a prescription of medical herbs named Shenmai Dihuang decoction(SDD) and observed its results from clinical investigation. Methods: We divide 60 patients of CRF into SDD group, western medicine (BaoQuanYangHuaDianFen) control group at random, each group has 30 patients. In the two groups, the clinical symptoms and the change of BUN, Cr, RBC, Mb in blood before and after treatment are observed respectively, at the same time, we monitor the ET-1 in blood of the SDD group , comparing it with normal blood donors group. Results: From the results of clinical research , We can see that SDD can not only alleviate symptoms of the patients effectively, but also improve anemia and the renal function of the patients dramatically. The total efficiency is 83.3% which is better thanthe control group (46.7 %) significantly. SDD can also cut down the concentration of ET~1 in the blood obviously. Conclusion: According to the results, we can conclude that SDD is an efficient recipe for CRF by improving the renal function and life quality of the patients effectively. The therapeutic mechanism may have much to do with the regulation of immunity, improvement of the blood circulation in kidney, and the descendent of ET-1 in blood.
