Clinical Study On The Insufficiency Syndrome Of Qi And Yin Of Chronic Glomerulonephritis And Effect On Plasma Endothelin Treated With Jiawei Shenqi Dihuang Decoction

Objective: In the study, the TCM theories of treatment according to syndrome differentiation were used to probe into the etiology,pathogenesis,management method and prescription of the insufficiency syndrome of qi and yin of Chronic Glomerulonephritis (CGN) , and to find out the efficient way of treating CGN. Based on years of clinical experiences, my tutor proposed that the main mechanism of CGN is based on the deficiency of lung, spleen and kidney. Wind-cold, damp-heat and blood stasis are important factors that effect the development of CGN. And that in the medium and late stages of CGN deficiency of qi and yin of spleen and kidney is common. So invigorating spleen and tonifying kidney, nourishing qi and yin, assisted with removing stagnation of blood are the main treatment principles of it. Guided by these principles, we made up a prescription of medical herbs named Jiawei Shenqi Dihuang decoction (JSDD) and observe its results from clinical investigation. Methods: We divide 60 patients of CGN into JSDD group and control group (ShenYanKangFuPian) at random, each group has 30 patients, which are treated by the same methods of modern medicine, such as descending blood pressure , promoting urination etc. In the same time Jiawei Shenqi Dihuang decoction is given to the JSDD group while ShenYanKangFuPian to the controls. In the two groups, the clinical symptoms , the nature of urine albumen, the ration of 24 hours' urine albumen and the change of BUN and Scr before and after treatment are observed respectively. In the meanwhile we monitor the ET-1 in the blood of the JSDD group. Results: From the results of clinical research ,we can see that JSDD can not only alleviate symptoms of the patients effectively, but also descend albuminuria and protect the renal function of the patientsdramatically. The total efficiency is 86.7% which is better than the control group (50.0%) significantly. JSDD can also cut down the concentration of ET-1 in the blood obviously. Conclusion: according to the results, we can conclude that JSDD is an efficient recipe for the insufficiency syndrome of qi and yin of CGN by protecting the renal function ,descending albuminuria, alleviating symptoms and improving life quality of the patients effectively. The therapeutic mechanicsm may have much to do with the regulation of immunity, enhancement of the blood circulation in kidney, improvement GFR, accelerating excretion of BUN, descendent of ET-1 in blood and restraining damnification of SOD.
