Objective To evaluate the expression and clinical significance of p27kipl and Retinoblastoma (Rb) protein in primary gallbladder carcinoma . Methods we study each expression of p27kipl and Rb protein ,the relationship between each expression and clinicopathological factors in primary gallbladder carcinoma by using SABC immunohistochemical staining in specimens of 42 cases of primary gallbladder carcinoma and 20 cases of chronic cholecystitis . Results (1)The positive expression rate of p27kipl protein and Rb protein in primary gallbladder carcinoma was 33.3%(14/42) and 71.4%(30/42),respectively,which were lower than benign lesion of gallbladder 90.0%(18/20) and 100%(20/20)(P<0.01, respectively). (2) The expression level of p27kipl protein in primary gallbladder carcinomas showed an obvious difference in the nevin's stage, histological grade ,depth of invasion, lymph node metastasis, (P<0.05 ,respectively). but not markedly associated with patient's age, tumor size and histological types .The expression level of Rb protein in primary gallbladder carcinomas was significantly related with lymph node metastasis and nevin's stage (P<0.05, respectively), but be not markedly associated with patient's age, tumor size, histological grade,depth of invasion and histological types (P>0.05, respectively). Log rank test showed that the p27kipl expression was significantly related with prognosis of the patients (P<0.05), and the prognosis of the patients with p27kipl positive expression was markedly better than that of those with p27kipl negative expression. Cox's regression indicated that p27kipl was an independent prognostic marker of gallbladder carcinomas .(3) There was a significantly inverse relationship between the expression of p27kipland Rb protein in primary gallbladder carcinoma. Conclusions (1)The abnormal expression of p27kipland Rb protein may be involved in the genesis and development of primary gallbladder carcinoma.(2) The expression of p27kiplwas significantly related with clinicopathological factors of primary gallbladder carcinoma. p27kipl protein may be used as a new novel tumor marker for primary gallbladder carcinoma . (3) The expression of p27kipl and Rb protein has significant inverse associations ,and may be coexist negative feedback regulator mechanism between them in primary gallbladder carcinoma.
