BackgroundAt present, the effectiveness of pre-pregnancy health care which has been carrying out in our country is not satisfied Thus, the incidence of birth defects is still high and it has increasingly become a public health problem by affecting the quality and health of the population. Therefore, it is imperative to strengthen the implementation and promotion of the existing pre-pregnancy health care services, to improve the service coverage rate and, meanwhile to explore more effective and conprehensive methods to enrich the content of pre-pregnancy health care services. Traditional Chinese medicine (TCM), a quintessence of china, has a long eugenics history, especially the development and the evolving of TCM theory in recent years, the constitution in TCM theory gradually formed and become an important part of the system of TCM. Meanwhile, this theory provided a theoretical basis for TCM applied in. the field of pre-pregnancy health care. Meanwhile, the Constitution in Chinese Medicine Questionnaire, with good reliability and validity, provides technical support for application of constitution in TCM in the field of pre-pregnancy health care. Developing TCM pre-pregnancy health care service will be helpful to reduce the incidence of birth defects and inprove the quality of new-born population. However, to date, TCM pre-pregnancy health care services has no position in the field of eugenics. Consequently, it is essential to realize people’s attitude and demand toward this service before carrying out the investigation, because the demand of people is the precondition of the government to formulate the relevant system.AimsThis study was taken from the perspective of demand, with women who had birth wish living in Beijing Pinggu district as research object, and then provided reference foundation to government to develop TCM pre-pregnancy health care services and to formulate the relevant system through investigating people’s demand toward this service and analyzing its influencing factorsMethods522 women of childbearing age who had fertility intention were investigated by using questionnaire. Investigation content included:(1)Basic situation; (2)Awareness rate of TCM pre-pregnancy health care knowledge; (3)Demand for TCM pre-pregnancy health care services; (4) Content of TCM pre-pregnancy health care knowledge and demand for acquiring way; (5) Content of TCM pre-pregnancy health care services knowledge and demand for services. Analyzing the factors that affecting demand for TCM pre-pregnancy health care knowledge and service by using chi-square test and stepwise logistic regression.Results1、Basic situation:In total,522 women were tested.95.40% participants with age between 20 to 30 years old; 78.74% women accepted at least technical secondary school or high school education; Career in the private enterprise (20.11%) and freelancers (31.42%); Family per capita monthly income was majority between 1000-3000 yuan, accounting for 62.45%; 73.37% people who live in a place where community health service centre providing TCM services。2、Recognition of TCM and influencing factors:94.06% participants recognized TCM; 82.95% people would choose TCM when they were ill. Chi-square test shows that the influencing factors of recognition are level of education and whether community providing TCM services t. Recognition rate is high when people with high level of education and live in a place where community health service centre providing TCM services. The factors influencing people’s choice of TCM treatment are incomes and whether the community providing TCM services. Those people who with low income and their communities providing TCM services will choose TCM treatment when they get sick.3, Awareness rate of pre-pregnancy health care knowledge and influence factors: Awareness rate of constitution impacting on pregnancy complications and pregnancy outcome are 77.01% and 79.12%, respectively. Constitution conditioning methods with TCM characteristics including diet nursing, meridian curing, traditional Chinese medicine health care exercises, awareness rate were 72.99%,58.62% and 53.64%, respectively. Single factor analysis showed the influence factors of awareness rate are:level of education, incomes, communities providing TCM services or not and recognition of scientificity of TCM.4、Demand for TCM pre-pregnancy health care services and influence factors: 92.91% people demand TCM pre-pregnancy health care services. Single factor analysis showed that the factors influencing demand for TCM pre-pregnancy health care services including:communities provide TCM services or not, recognition of scientific of TCM, choice of TCM treatment, and awareness rate of TCM pre-pregnancy health care knowledge. Multi-factor analysis was made by using stepwise logistic regression, and the result showed that the higher the recognition and awareness rate, the higher the demand rate.5、TCM pre-pregnancy health care knowledge and acquiring way:Compared with other traditional Chinese medicine nursing methods,66.86% participants knew TCM diet nursing knowledge.6、Content and institution of TCM pre-pregnancy health care services:54.02% people hope TCM pre-pregnancy health care services providing consultation of TCM pre-pregnancy health care knowledge. Maternity and child care centers and community health service centers are demanded as the institutions providing TCM pre-pregnancy health care services,60.15% and 37.55%, respectively.ConclusionPrincipal Findings1、Participants were with low awareness rate of the constitution conditioning methods, meridian curing and traditional Chinese medicine health care exercises. The rates were 58.62%,53.64%, respectively.2、People who had the demand for preconception care services of traditional Chinese Medicine in the study account for 92.91% of the total number.3、The influencing factors of the demand for TCM pre-pregnancy health care services are including direct factors and indirect factors. Direct factors are the attitude toward TCM and t awareness rate of pre-pregnancy health care services knowledge. Indirect factors including educational level, income, choice of TCM treatment, whether community health service centers provide TCM services. Indirect factors affect the requirements through direct factors.4. Community health service centers which providing services can affect people’s attitude toward traditional Chinese doctors, choice of TCM treatment and awareness rate of TCM knowledge. It is helpful to propaganda TCM culture by expanding TCM coverage in basic health service centers.5、Pinggu District has the mass base to develop TCM pre-pregnancy health care services from the perspective of demand:recognition rate of TCM accounted for 94.06%, demand rate of TCM pre-pregnancy health care services accounted for 92.91%.AdvantagesAlthough TCM pre-pregnancy health care services which based on constitution in TCM has received good results, but it is not widely carried out in the crowd. The study of demand for TCM pre-pregnancy care services was not yet reported. This study was taken in women of childbearing age and investigated people’s demand toward TCM pre-pregnancy health care services. The purpose of this research is to provide reference foundation to government to develop TCM pre-pregnancy health care services and to formulate the relevant system.
