Part I:A community demand research on TCM intervention model of colorectal cancerObjective:A preliminary understanding of the awareness and demand for Chinese medicine of target population through surveys, provide the basis for the next step to establish TCM intervention model throughout the process of colorectal cancer.Methods:124people are designed to different structure questionnaires called’the needs of colorectal cancer prevention knowledge questionnaire’which are filled out by way of on-site survey, who are made up of specialists and nurses coming from Xiyuan Hospital, doctors and community management staffs coming from HaidianChinese medicine hospitals and Shuangyushu community health centers, and patients pathological diagnosedas colorectal cancercoming from Xiyuan Hospital, Haidian Hospital and the community under jurisdiction.All data is processed by SPSS20.0 statistical software.Results:115 valid questionnaires are collected of total given 124.Survey results showed that:only 20% doctors can fully instruct and educated the prevention knowledge of colorectal cancer; 85% of health care workers think about the work is more appropriate to put the community hospital;94% of colorectal cancer patients prefer to get cancer-related prevention and treatment of hospital-acquired knowledge of nursing intheir own communities;66.7% of the community physicians expressed admissions of patients with colorectal cancer or at high risk of colon cancer in their daily work, but only 26.7%of the people expressed a certain degree of understanding in colorectal cancer prevention and treatment of nursing knowledge;86.7% of community managers expressed needs of the community physician knowledge and training of cancer;93.3% of doctors said they have desires about science education on colorectal cancer patients and high-risk groups in the area of jurisdiction; only 6 percent of colorectal cancer patients are willing to choose the community hospital; up to 100% of patients indicated universal needs of publicity and education on the knowledge of colorectal cancer; on the realization way of training and education, choosing healthy seminars organized by various medical nstitutions accounted for 27.8%, selectingprofessional network platform established by professional medical personnel and tumor-associated organization accounted for 55.7%, selecting the relevant science books accounted 16.5%; 14.8% of people choose to Western medicine knowledge of colorectal cancer;22.6% of people choose lifestyle guidance; 15.6% of people choose to follow-up of colorectal knowledge; 47.0% of people are more concerned with colorectal cancerrelated TCM’s treatment and recuperation knowledge.Conclusions:Oncology professional doctors, nurses, community physicians, community management and colorectal cancer patients think it’s necessary to realize the training and educationintervention programs of colorectal cancer in the community. When talking about the implementation methods, most people prefer to accept the professional network platform, which should be divided into 3 different parts respectivelyfor doctors, patients and high-risk groups with different training and education programs. For the contents of the program, most focusing on TCM’s preventive care, treatment, rehabilitationand aftercare. It’s also important to take into account the relevant prevention knowledge of Western medicine, regular follow-up, scientific screening way, healthy lifestyle guidance and so on. This study provides an important basis for the next preparation on TCM intervention model throughout the process of colorectal cancer.Part Ⅱ:The establishment of community TCM intervention model of colorectal cancerObjective:Based on the previous needs investigation, working out the community TCM intervention model of colorectal cancer combined with the actual needs.Methods:According to preliminary needs, using the method of’expert consultation’, which organized clinical experts including methodology specialists, epidemiologists, health administration experts and community health workers to repeat three rounds of discussions and interviews, works out the basic principles,methods and structuresof the model.Results:The formed TCM intervention programs, after repeated discussions of colorectal cancer experts, mainly includes three parts:The first part is related to the training of community physicians about professional medical knowledgeassociated with colorectal cancer; the second part of the mission for colorectal cancer patients; the third part is mainly aimed at high-risk groups of colorectal cancer missionary content.Conclusions:The established TCM intervention programs, after repeated discussions for different groups of three parts,show its strong pertinence and practicability and provide the basis for the next step to establish the colorectal cancer management model and its application research.Part Ⅲ:Theconstruction and applicationresearch of chronic disease management information systemsonTCMintervention of colorectal cancer in the community.Objective:To establish and verify the chronic disease management information systems on TCM intervention of colorectal cancer in the community. Finding out the situation and problems of chronic management of colorectal cancer, so as to improve the quality of it.Methods:Withanalyzing the developmentscenarios, technology and network servers of colorectal cancer chronic disease management information system with Programming and Network engineers, the system is initially established. The same user fills out the electronic questionnairesbefore and after training or education andwe finally calculate and analysis the awareness of various types knowledge on cancer prevention and control. The data is analyzedby SPSS20.0 statistical software. Using the percentage as the general statistical description and the x2 test.Results:Shuangyushu community physicians fill in 40 partsof valid e-questionnaires before and after the training.Statistical results showed that:the awareness about how to identify high-risk groups of colorectal cancer correctly, before and after training, is respectively50% and 97.5%; the awareness of colorectal prevention knowledge is respectively 17.5% and 92.5%; the awareness of TCM knowledgeon colorectal canceris respectively37.5% and 90.0%; the recognition ofTCM prevention and treatment of colorectal cancer has increased to 95.0% from 57.5%. Statistical results are tested by chi-square test (P<0.001). There are significant differences in the results.Statistical results of 50 patients’awarenessbefore and after the education show that:the awareness around healthy lifestyles isrespectively20% and 84%; the awareness of thefollow-up principle of colorectal cancer isrespectively 24% and 96%; the awareness of TCM prevention knowledge on colorectal cancer is 16% and 86%; therecognition ofthe function of TCM prevention and treatment on colorectal cancer is 24% and 78%; themethod acceptance, using TCM to treat and prevent the colorectal cancer, is respectively 58% and 98%.Statistical results are tested by chi-square test (P<0.001). There are significant differences in the results.Statistical results of 50high-risk groupsand general population, before and after the direction, and education show that:the awareness of colorectal cancer prevention knowledge is respectively20% and 92%; the awareness of early symptoms of colorectal cancer is 22% and 98%; the awareness of proper screening way is 18% and 94%; the awareness of Chinese medicine for the prevention of health knowledge missionary awareness around 28% and 90%; themethod acceptance, using TCM to treat and prevent the colorectal cancer, is respectively68% and 94%. Statistical results are tested by chi-square test (P <0.001). There are significant differences in the results.Conclusions:Thechronic disease management information systems on TCM intervention of colorectal cancer in the community can significantly improve the community physicians on the professional prevention and treatment of colorectal cancer, can improve the awarenessof prevention knowledge among colorectal cancer patients, the general population and high-risk groups of colorectal cancer. It’s helpful at the establishment of colorectal cancer prevention and treatment professionalteam in the community. It alsoimproves the ability of community service and public awareness for colorectalcancer.
