Health equity is a long-term major problem faced by the development of human society,and it is also the main public health goal of various countries in building a medical and health service system for a long time.Especially in the Qinghai-Tibet region(Qinghai Province,Tibet Autonomous Region),the development of medical and health services is relatively lagging,and it is difficult for residents to effectively match the medical needs and service supply,and the spatial distribution is uneven.These factors have affected the fairness of medical and health services in the Qinghai-Tibet region.developing.Therefore,the research on the spatial equity of medical and health services in the Qinghai-Tibet region will further promote the improvement of the quality and efficiency of medical and health services in the Qinghai-Tibet region,which has important theoretical and practical significance for promoting the sustainable development of health.In order to solve the structural problem of the equity of medical and health services in the Qinghai-Tibet region,from 2018 to 2021,this research has obtained relevant research data on the development of medical and health services in the Qinghai-Tibet region through long-term field work in the Qinghai-Tibet region,and carried out quantitative or case studies.Firstly,The research sorts out the research summary and theoretical basis,sorts out the existing research data,and understands the frontier theories of the equity of medical and health services.Secondly,Secondly,based on the actual conditions of medical and health services in the Qinghai-Tibet region,this research combines existing research to construct an analytical framework for the research on equity in medical and health services in the Qinghai-Tibet region,and establish the realistic and theoretical connection between space and equity.Thirdly,this study analyzes the geographic spatiality of medical and health services in the Qinghai-Tibet region,and uses a gravity model to form a spatial correlation structure of medical and health services in the Qinghai-Tibet region.Through overall network analysis,individual network analysis,CONCOR analysis,and QAP regression analysis,a total of 15 regions in the Qinghai-Tibet region are analyzed.City and prefecture for analysis.The data results discuss its location in the network,spatial characteristics,and "center-periphery" conditions,and form factors affecting geographic spatial fairness.Then,through the "Xining Model" and the case of Tibet’s group-style medical counterpart support,this research explores the elements that affect the fairness of the medical and health service system in the Qinghai-Tibet region from the perspective of the fairness of administrative space and the fairness of cross-domain governance.At last,based on the results of the empirical analysis,the research draws conclusions and forms a policy direction for improving the equity of medical and health services in the Qinghai-Tibet area.The results of the study show that,on the one hand,from the perspective of geographic space,the medical and health services in the Qinghai-Tibet region have obvious benefits or spillover effects in terms of individual associations and relationships between sectors.Differences in economic levels,differences in medical facilities,differences in human resources,and geographic proximity are the main factors that affect the fairness of geographic space.According to the results of the concentration index,the spatial effects of spatial differences have affected the fairness of medical and health services in the Qinghai-Tibet region,and unfair medical and health services are presented in terms of population distribution,geographic distribution and effect structure.On the other hand,from the perspective of institutional space,the“Xining Model” and the case of group medical treatment show that the adjustment of the main body or cross-domain diversified cooperation,policy innovation,interest relations,cooperation costs and other factors are all changes in the health governance structure and guarantees the fairness of residents.The way to obtain medical and health services will ultimately promote the fairness of the development of medical and health services in the Qinghai-Tibet region.The innovations of this research as follows: First,this research establish an integrated spatial fairness analysis framework;Second,this research use gravity models and social network methods to establish a spatial fairness structure;Third,this research establish the measurement dimensions and analysis of the spatial fairness level indicator system. |
