Since the 18 th CPC National Congress,China has historically eliminated absolute poverty through the implementation of targeted Poverty Alleviation Policies,completed the goal and task of poverty alleviation as scheduled,and made important contributions to the cause of poverty reduction in the world.However,the relatively poor rural populations still face the risk of returning to poverty,of which the main reason is due to illness.Due to their poor health level and low health literacy,middle-aged and elderly people are at greater risk of returning to poverty due to illness,which is particularly worthy of attention.The key to preventing people who have been lifted out of poverty from returning to poverty due to illness is to ensure their medical needs and improve the utilization of medical services.The Party Central Committee and the State Council also attach great importance to this issue.In the period of targeted poverty alleviation,a series of policies including health poverty alleviation are implemented to ensure the medical needs of poor groups and promote the utilization of their medical services.To consolidate the achievements of poverty alleviation,General Secretary Xi Jinping requested that the relevant policies of targeted poverty alleviation should remain stable during the rural revitalization stage.However,rigorous empirical research is still lacking on whether these policies increase the level of medical service utilization among the poor.Existing literature mainly focuses on analyzing the effect of precision poverty alleviation on poverty reduction and income increase,and rarely assesses the impact of targeted Poverty Alleviation Policies on the utilization of medical services among rural middle aged and elderly people.Therefore,based on the four-phase panel data of the China Health and Retirement Tracking Survey and a randomized intervention experiment,this study uses the research methods of econometrics and experimental economics to evaluate the impact of targeted poverty alleviation and special poverty alleviation actions on the utilization of medical services for rural middle-aged and elderly people,and analyze its mechanism of action,and finally provides decision-making reference for optimizing poverty alleviation policies,consolidating poverty alleviation achievements and realizing rural revitalization.To achieve the above goals,the research content of this study mainly includes the following three aspects:(1)To analyze the current situation of medical service utilization of the rural middle-aged and elderly people,the influencing factors of their medical service utilization,and the reasons that hinder their use of medical services.(2)To evaluate the impact of targeted poverty alleviation policies on outpatient and inpatient medical services,and to test the three mechanisms of action.(3)Taking the special poverty alleviation action of the Cataract Bright Project in targeted poverty alleviation as an example,based on a random intervention policy simulation experiment,the impact of the special poverty alleviation action on medical services for the middle-aged and elderly in rural areas was evaluated.The main conclusions of this study are as follows:(1)The self-rated health of rural middle-aged and elderly is poor,and there is still a certain proportion of patients who have not been treated after an illness.Among them,economic difficulties are the main reason that hinders their use of medical services.(2)The targeted poverty alleviation policy significantly increases the possibility and medical expenditure of inpatient medical service utilization for rural middle-aged and elderly people but has no significant impact on the utilization of outpatient medical services.(3)In the short term,the targeted poverty alleviation policy has not had a significant impact on individual health,but by increasing the medical reimbursement ratio and income level,it has promoted the utilization of medical services for rural middle-aged and elderly and released their medical needs.(4)Although the special poverty alleviation action of the Cataract Bright Project has significantly increased the rate of medical treatment and surgery of cataract patients in rural areas,the effect is limited.Our further analysis found that apart from economic difficulties,the lack of sufficient health knowledge among rural patients is an important reason to hinder their utilization of medical services.Based on the above findings,this study puts forward three policy suggestions:(1)Continue to maintain the stability of the assistance policy for relatively poor groups to ensure that their medical service needs can be met on time.(2)Further explore other possible ways to improve the utilization rate of outpatient services,to avoid minor illnesses causing major illnesses.(3)Strengthen the health education of the relatively poor population,improve their health literacy,and thus maximize the impact of poverty alleviation policies on the utilization of medical services.
