Objective: Dyslipidemia is a chronic disease of lipoprotein metabolism and a risk factor for diseases such as heart,brain,liver and kidney.Dyslipidemia has a high prevalence in my country,but the awareness rate,treatment rate and control rate are relatively low.Therefore,it is urgent to carry out the health management of dyslipidemia.This study aims to explore the health management service model of dyslipidemia,intervene dyslipidemia based on the integrated health management service model,and evaluate the effect.Materials and methods: The random cluster sampling method was used to select highrisk individuals aged 18-79 who visited 1-9 health service centers in five urban areas of Shenyang City,and were collected through questionnaire survey,physical examination and biochemical index detection.Data,SPSS 22.0 software was used to conduct univariate and multivariate analysis of risk factors for dyslipidemia.Then,through the literature method,semi-structured interview and Delphi method,a dyslipidemia management model was constructed based on the framework of a new integrated health management model.Finally,patients with dyslipidemia in 5 urban areas of Shenyang City were selected and randomly divided into experimental group and control group.The new health management model was applied for 2 cycles of 24 months of intervention.Data were collected through questionnaires(health risk assessment,national resident health literacy survey,health status survey SF-36 and doctor-patient relationship survey),physical examination and biochemical index detection,and repeated measures analysis of variance was used to compare the effects of health interventions.Results: A total of 2362 people were investigated in this study,of which 42.3% were male and 57.7% were female.2362 valid questionnaires were recovered,and the effective recovery rate was 100%.From the aspects of sociodemographic factors,behavioral risk factors,biological risk factors,and psychological factors,univariate and multivariate analyses were carried out to analyze the related conditions of dyslipidemia among highrisk groups of chronic diseases in Shenyang.The results of multivariate analysis showed that age,BMI,cooking oil intake,annual family income,sedentary time,hypertension,diabetes and coronary heart disease were the influencing factors of dyslipidemia.The number of patients with dyslipidemia was 934,and the prevalence rate was 39.5%.Among them,there were 383 males,with a prevalence rate of 38.3%;551 females,with a prevalence rate of 40.5%.Then,based on the Delphi method,this study combines domestic and foreign research foundations and expert opinions to construct five first-level indicators of "team building","information collection","disease prevention","health protection" and "health promotion".Dyslipidemia management model with 17 secondary indicators.Finally,the new health management model is used to carry out intervention research on patients with dyslipidemia.A total of 898 patients with dyslipidemia were intervened,of which 497 were in the experimental group,accounting for 55.3%;401 were in the control group,accounting for 44.7%.Before and after the intervention,blood lipid levels,blood pressure,blood sugar,BMI levels,behavioral risk factors,residents’ health literacy levels,quality of life,and patient satisfaction were significantly changed.The new health management model can significantly reduce patients’ triglyceride(TG),total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)levels,while increasing high-density lipoprotein cholesterol(HDL-C)levels,and with The longer the intervention time,the more obvious the effect;the new health management model can reduce blood pressure,BMI level,smoking rate and unhealthy eating habits in patients with dyslipidemia,while fasting blood glucose and drinking rate have no significant changes;Levels,quality of life and patient satisfaction have improved to varying degrees.Conclusion: 1.The influencing factors of dyslipidemia in high-risk groups of chronic diseases in urban areas of Shenyang are age,annual income,BMI,cooking oil intake,sedentary time,hypertension,diabetes and coronary heart disease.2.Based on the Delphi method,a "dyslipidemia management model based on a new integrated health management framework" has been constructed.3.The application of dyslipidemia health management model to intervene in dyslipidemia patients has obvious effect,which provides a reference for the management of dyslipidemia patients.
