| Irritable bowel syndrome(IBS)is a kind of common functional gastrointestinal disease.IBS has four sub-types,one of which is irritable bowel syndrome-constipation(IBS-C).Because the pathogenesis of this disease is unknown,and it is often complicated with psychological and mental symptoms,there also lack effective treating measures clinically.The treatment of western medicine mainly directs to the disease and the treatment with traditional Chinese medicine is somehow advantageous.The acupoint catgut embedding therapy of traditional Chinese medicine has obvious features in treating IBS-C.This essay researches and discusses the treatment effect and operating mechanism of the acupoint catgut embedding therapy in treating IBS-C in two parts:clinical research and experiment research.Clinical partPurpose:To observe the clinical effect of the acupoint catgut embedding therapy by the researching method of prospective random controlling.Method:Calculate the sample content with formula method and dividing 94 patients into treatment group and control group(each group contains47patients)randomly.The acupoint catgut embedding therapy is used in the treatment group,and the selected acupoints include Tianshu,Zhongwan,Zusanli,Piyu,Ganyu and Dachangyu;the catgut embedding is performed once per 2 weeks and it is performed 2 times altogether;Calcium Polycarbonphil Tablet is taken by the patients in control group,and the course of treatment lasts for four weeks.Results:All the 94 patients come from the outpatient service of proctology department of this hospital.The patients in two groups were compared in terms of sex,age distribution,disease course distribution,illness state severity and symptoms of traditional Chinese medicine,and no differences are shown(P>0.05);the homogeneity is good and the data can be compared.(1)In terms of general treatment,the total effective rate of the treatment group and control group are 97.87%and76.60%respectively.The general effective rate of the treatment group is obviously higher than that of the control group,P<0.01,and it is significant,indicating the treatment effect of the treatment group(the group of catgut embedding therapy)is obviously better than that of the control group(the group of Calcium Polycarbonphil Tablet).(2)The scores of the treatment group in terms of the main syndromes before and after treatment,including time interval for excretion,excreting difficulty,excreta feature,excreting time and abdomen pain,decreases obviously than before;P<0.01 and it is statistically significant,and it indicates that the acupoint catgut embedding therapy can obviously alleviate the five main symptoms of IBS-C.(3)The scores of 9 accompanied symptoms of the treatment group before and after treatment decreases than before,they are abdominal distention,parched mouth and scorched tongue,flustering and lacking in strength,insomnia and dreaminess,fullness chest and hypochrodrium,belching and sighing,sweating and hard breathing,inappetence as well as anxiety and irritability;P<0.01 and it is statistically significant,and it indicates that the acupoint catgut embedding therapy can obviously alleviate the accompanied symptoms of IBS-C.(4)The scores of the main symptoms of time interval for excretion,excreting difficulty,excreta feature,excreting time and abdomen pain of the two groups decreased comparing with that in the control group;P<0.01 and it is statistically significant,and it indicates that the acupoint catgut embedding therapy can alleviate the five main symptoms of IBC-S better than the control group;(5)for the accompanied symptoms of the two groups before and after the treatment,the scores of abdominal distention,parched mouth and scorched tongue,inappetence and anxiety and irritability in the treatment group is obviously lower than those in the control group;P<0.01 and it is statistically significant;the scores of other accompanied symptoms don’t show differences between the treatment group and control group and it indicates that the acupoint catgut embedding therapy can alleviate the accompanied symptoms of IBS-C of abdominal distention,parched mouth and scorched tongue,inappetence and anxiety and irritability better than those in the control group;and there is no difference between two groups in terms of other accompanied symptoms.Conclusion:(1)In the group of acupoint catgut embedding and the group of Calcium Polycarbonphil Tablet,the treatment of IBS-C are both effective,and the situation of the treatment group in terms of general treatment effect,alleviation of main symptoms and alleviation of partial accompanied symptoms are is better than those in the control group;(2)the acupoint embedding therapy has simple operation,no side effects and determined treatment effect,and it can be popularized in a further step.Experiment part:Purpose;By establishing IBS-C animal model,the operators can observe the treatment effects of the acupoint catgut embedding therapy as well as the effect of the acupoint catgut embedding therapy to 5-HT and CRF in the animal serum group and intestinal group of IBS-C experiment,and discuss the mechanism of IBS-C in a further step.Method:Using the methods of gavage with icy water,tail clamping for stimulation and fixing by constraining to create the model of the experimentally-used rats with the symptoms of insufficiency of the spleen,qi stagnation and liver depression of IBS-C,and by observing the general status and excreta of the rat as well as ink transferring test and proctectasis test to prove that the constipation model is successfully copies.The acupoint catgut embedding therapy is used in the treatment,and the mosapride treatment is used in the control group.Results:(1)The excreta number within 24 hours;the excreta number of the embedding group is larger than that of the control group;P<0.05,and it indicates that the treatment is effective;the excreta number of mosapride group is larger than that of the model group;P<0.05,and it indicates that the treatment is effective;the excreta number of the embedding group is larger than that of mosapride group,and it indicates that the treatment effect of acupoint catgut embedding is better than that of mosapride group;(2)the wet weight of excreta within 24 hours,the embedding group is larger than that of the model group,P<0.05,and the treatment is effective;the mosapride group is larger than the model group,P<0.05,and the treatment effective;the embedding group is larger than mosapride group,and it indicates that the treatment effect of the acupoint catgut embedding group is larger than mosapride group;(3)the wet weight of excreta within 24 hours,the embedding group is larger than that of the model group,P<0.05,and the treatment is effective;the mosapride group is larger than the model group,P<0.05,and the treatment effective;the embedding group is larger than mosapride group,and it indicates that the treatment effect of the acupoint catgut embedding group is larger than mosapride group;(4)the progression rate of ink transferring test,the embedding group is larger than IBS-C model group,P<0.05,the treatment is effective;mosapride group is larger than model group,P<0.05?the treatment is effective;the embedding group is larger than that of mosapride group,and it indicates that the treatment effect of the acupoint catgut embedding group is better that of mosapride group;(5)the effect of the acupoint catgut embedding group to 5-HT of the animals of IBS-C experiment:comparing with lIB S-C model and normal group,the5-HT value in the serum and intestinal tissues increases obviously,P<0.05,and it is statistically significant,and it indicates that 5-HT might be the factor causing IBS-C.Comparing with acupoint embedding group and IBS-C model group,the value of 5-HT in serum is lower than that of IBS-C model group,P<0.05,and it indicates that the treatment is effective;comparing with mosapride group and IBS-C model group,the value of5-HT in serum is lower than that of IBS-C model group,P<0.05,and it indicates that the treatment is effective;comparing with acupoint embedding group and mosapride group,the value of 5-HT in serum is lower than that of mosapride group,P>0.05.and it indicates that there is no difference between acupoint embedding group and mosapride group;(6)the effect of the acupoint embedding method to CRF of the experimentally used animals for IBS-C:comparing with IBS-C model group and normal group.the CRF value in serum and intestinal tissues increase obvious.P<0.05.and it is statistically significant,it indicates that CRF might be the factor causing IBS-C.Comparing with acupoint embedding group and IBS-C model group,the value of CRF in serum is lower than that of IBS-C model group,P<0.05,and it indicates that the treatment is eff’ective;comparing with mosapride group and IBS-C model group,the value of CRF in serum is lower than that of IBS-C model group,P<0.05,and it indicates that the treatment is effective;comparing with acupoint embedding group and mosapride group,the value of CRF in serum is lower than that of mosapride group,P>0.05.and it indicates that there is no difference between acupoint embedding group and mosapride group.Conclusion:The acupoint catgut embedding therapy is effective to the treatment of IBS-C animal model,and it is better than that of the control group using mosapride;5-HT and CRF might be the pathogenic factors of IBS-C,and the both therapies can affect it,and it also indirectly indicates the mechanism of the IBS-C treatment with acupoint catgut embedding method. |
