Background and purpose: Irritable bowel syndrome (IBS) is a common functional disease, abdominal pain, abdominal discomfort as the main symptoms, and accompanied by changes in bowel habits and characteristics, in which diarrhea (IBS-D) is the most common. The clinical manifestations of IBS-D are different, which may occur alternately and continuously, and the symptoms often occur repeatedly, which can seriously affect the quality of life of patients. The medicine is commonly used in the treatment of IBS-D traditional Chinese medicine prescription, especially for patients with liver depression and spleen deficiency, which is worthy of further study. The purpose of this study is:1. To explore the possible mechanism of Tongxieyaofang prescription of traditional Chinese medicine treatment of liver stagnation and spleen deficiency type IBS-D ;2 .To explore the significance and value of plasma 5- HT and MOT levels in IBS-D in liver and spleen deficiency;3. Objective to investigate the pathogenesis of IBS-D and the relationship between plasma 5-HT and MOT levels and the diagnosis of IBS-D.Method: In this study, according to the relevant standards, a reasonable clinical research design. Combined with IBS-D diagnostic criteria of Rome actual situation, and also meet the diagnostic criteria of syndrome of liver depression and spleen deficiency, 40 cases at the age of 18-60 years old patients, in patients informed consent, patients were given Tongxieyaofang granules 10g / bag, 1 bag each time, with warm water, 2 times a day,to observe the curative effect of continuous after 4 weeks of therapy, 20 cases with another healthy control group selection criteria of healthy subjects. According to the guiding principles of clinical research on new drugs of traditional Chinese medicine,the curative effect evaluation criteria were determined, and the clinical symptoms of 40 patients with liver depression and spleen deficiency type IBS-D were evaluated before and after treatment.Plasma 5-HT and plasma MOT were measured before and after treatment in patients with liver depression and spleen deficiency type IBS-D,and plasma 5-HT and plasma MOT were measured. The changes of clinical symptoms and the changes of plasma 5-HT and plasma MOT levels in patients with IBS-D before and after treatment were observed and compared with those of healthy subjects.Results: 1. There was significant difference in plasma 5-HT level between IBS-D patients and normal subjects (P<0. 01). There was significant difference in plasma MOT level between IBS-D patients and normal subjects (P<0. 01), and there was statistical difference between the two groups. ; 2. The prescription of traditional Chinese medicine prescription in the treatment of pain relief in patients with TCM liver stagnation and spleen deficiency type IBS-D, can make the plasma 5-HT level was significantly decreased (P<0. 01), and plasma MOT levels were significantly decreased (P<0.05 two),compared with before treatment were statistically significant; 3.There was no significant difference in plasma 5-HT level between the two groups (P>0.05), compared with the healthy control group(P>0.05), and the plasma MOT level was not significantly different from that of the healthy control group; 4. The prescription of Tong Xie prescription in the treatment of patients with IBS-D liver and spleen deficiency can significantly improve clinical symptoms of patients, the clinical cure in 10 cases accounted for 25%, 12 cases markedly effective in 13 cases, accounting for 30%, accounting for 32. 5%, invalid in 5 cases, accounting for 12. 5%, the total effective rate was 87.5%, compared with the healthy control group, the statistical difference (P<0. 01).Conclusion: 1. Tong Xie Yao Fang can obviously improve the clinical symptoms of IBS-D patients with liver stagnation and spleen deficiency syndrome, and have a comprehensive therapeutic effect on IBS-D patients with liver depression and spleen deficiency syndrome;2. Tong Xie Yao Fang can significantly reduce the level of plasma 5-HT and MOT in patients with IBS-D liver depression and spleen deficiency; 3. 5-HT and MOT in IBS-D patients with liver depression and spleen deficiency were significantly increased, and the level of plasma 5-HT and IBS-D could be used as routine assistant examination. However, it still needs further study to determine the numerical range of laboratory examination. |
