BackgroundLung disease is the most common diseases in newborn and children and the first cause of death in children younger than five years old.Therefore,diagnosing and monitoring lung disease are extremely important.The chest radiography(CR)and/or computed tomography(CT)are major means of diagnosing lung disease all the time,but they have their own inevitable shortcomings.With deepening recognization about lung ultrasonography in recent years,lung ultrasonography can make up for their deficiencies.Lung ultrasonography can quickly find the lung lesions and help physician timely adjust the strategy of treatment.But lung ultrasonography was still not widely used in newborn and children,especially in our country.Accordingly,our research aim at studying the diagnosis value of lung ultrasonography in pulmonary disease of newborn and children.Partl Lung ultrasonography in normal newborn and childrenObjective To study the ultrasonic features of normal lung in newborn and children.Methods All of 795 persons including neonates and children underwent lung ultrasonography exams.Results The ultrasonic signs of normal children included pleural-line,A-lines and excluded B-lines and other abnormal ultrasonic signs.The ultrasonic signs of normal neonatal pulmonary main were pleural line,A-line,B-lines or comet tail sign.B-lines of 713 neonates(95.4%)could be see within three days after birth and 256 neonates(34.3%)could be detected within three days to one week after birth.19 casescould be found with B-lines within one to two week after birth.Conclusions B-lines could be found in neonatal pulmonary for a long time,the existing time of B-lines was related to neonatal physiological conditions.Part 2 The value of Lung ultrasonography in neonatal pulmonary diseaseObjective To study the diagnosis value of lung ultrasonography in neonatal pulmonary diseases.Methods All of 2658 neonates with lung diseases included in this study underwent lung ultrasonography exams.Results The main features of neonates with lung disease on LUS were as follows:pleural-line abnormalities,absence of A-lines,interstitial syndrome,lung consolidation,pulmonary edema and lung pulse,etc.These abnormal features were reduced or eliminated on Lung ultrasonography as neonatal conditions improved.Conclusions LUS is a good way for the diagnosis of pulmonary disease and has the particularly clinical value for the diagnosis and identification of RDS and TTN,diagnosis pulmonary atelectasis,discrimination of causes of LTOP in premature infants.Part 3 Diagnostic value of lung ultrasonography in children lung diseasesObjective To study ultrasonic diagnostic value in lung diseases of children.Methods 137children in pediatric intensive care unitunderwent lung ultrasonography exams.Results The main ultrasonic manifestations of lung disease in pediatric intensive care unit were pleural-line abnormalities,absence of A-lines,interstitial syndrome,lung consolidation,etc.In the cases of failure in evacuating the respirator,lung ultrasonography easier found the pulmonary lesions than X-ray.Conclusions lung ultrasonography had accurate diagnosis in lung diseases,especially had more practical value for the children who were failure in evacuating the respirator?Part4 Lung ultrasonography compared with chest X-ray in diagnosis pulmonary diseaseObjective Tocompared Lung ultrasonography diagnosing pulmonary disease with chest X-ray.Methods 1692 cases of newborn and 137 cases of children received the exams of lung ultrasonography and chest X-ray.Results Both in the neonatal intensive care unit and pediatric intensive care unit,the results of lung ultrasonography and chest X-ray in lung diseases had significantly statistical difference.Conclusions Lung ultrasonography easierly found interstitial syndrome,atelectasis andhidden lesions than traditional X-ray in patients with lung diseases.Lung ultrasonography had greatly beneficial to the clinical diagnosis and treatment.
