| Stroke is the most common clinical disease with high incidence, high death rate and high mutilation rate characteristics. With the development of medicine, stroke mortality has been a certain control, but most survivors will appear such as limb weakness, adverse events, memory decline and other sequelae. Shoulder hand syndrome after stroke hemiplegia after stroke patients is considered to be the most common complications after stroke, is gradually appear shoulder joint movement disorder, ipsilateral concurrent elbow/wrist pain and movement disorder, a syndrome, serious influence patient’s quality of life. The clinical study showed that treatment with acupuncture has obvious advantages to relieve pain and upper limb swelling shoulder hand syndrome. Early and aggressive treatment can effectively prevent and reduce the incidence of shoulder hand syndrome. However, how to combine traditional Chinese medicine acupoint sticking to maximize efficacy, there is no further research. This study based on the acupoint application of Chinese medicine and rehabilitation training, the effect of electroacupuncture combined with acupoint to treat this disease applicator, to seek a method on shoulder hand syndrome is more complex therapy effectively, make the main and collateral channels acupoint therapy traditional to better play its unique advantage of the treatment.ObjectiveThis study adopts randomized study, the Fugl-Meyer of upper motor function score, activities of daily living scale, MPQ pain scale and SF-36quality of life assessment method, to study the effect of Electroacupuncture plus Acupoint Application of clinical effect of the treatment of shoulder hand syndrome after stroke, for the clinical treatment of shoulder hand syndrome after stroke provide more clinical data, application of acupuncture and acupoint application main and collateral channels acupoint therapy in the treatment of this disease.MethodsChoose the inclusion criteria and exclusion criteria of shoulder hand syndrome after stroke patients from Taiwan MingShi traditional Chinese medicine clinic, using simple random method, the random number table (SAS statistical analysis system). Collecting qualified shoulder hand syndrome after stroke patients were grouped according to the distribution of income, with random digit, electro acupuncture combined with acupoint application therapy group of38cases (treatment group), electro acupuncture group of38cases (control group). All of the patients were in accordance with established treatment regimens for treatment,1times per day, continuous treatment for10to1courses, two courses of treatment, and were followed up for a month. Finally, through the changes of the two groups were compared in different periods of various indicators of the effect (such as:activities of upper limb motor function, daily life ability of Fugl-Meyer (ADL, Barthel), pain assessment scale (MPQ) and quality of life (SF-36)), the observation scale statistical data, statistical analysis, comparison of clinical symptoms the results of quantitative data, to compare the clinical efficacy.Results1. This study collected in accordance with the inclusion criteria and exclusion criteria of the80cases,2cases were removed from the end,1cases obtained complete loss,77cases, among them,39cases in the treatment group,38cases in the control group. Before treatment in two groups in age, course of disease, gender, education, upper limb motor function evaluation, activities of daily living assessment, pain measurement and SF-36integral, there was no significant difference (P>0.05).2. Upper limb movement function:statistical data of two good treatment of upper limb movement function in patients with the control group, the total effective rate was86.84%, cure has3cases (7.89%), effective14cases (36.84%),16cases were effective (42.11%), invalid5cases (13.16%), no worse cases; the total effective rate in treatment group was94.87%, cure has7 cases (17.95%), effective17cases (43.59%),13cases were effective (33.33%),2cases (5.13%), no worse case. The function of the treatment group patients with upper limb motor was better than the control group(P<0.05). The treatment group was ideal and quick. The increase in the electroacupuncture acupoint on the basis of the paste, can obviously improve motor function of upper limb shoulder hand syndrome with integrated.3. In the improvement of activities of daily living in patients of the two groups of data, the results suggest two types of therapy can effectively improve the daily living ability of the patients. Among them, the control group the total effective rate was78.95%,10cases were cured (26.32%), effective in20cases (52.63%), invalid8cases (21.05%), deterioration in0cases; the total effective rate in treatment group was82.05%,15cases were cured (38.46%), effective in19cases (43.59%), invalid7cases (17.95%) in0cases, deterioration. The comparison, the efficacy of the treatment group is better than control group. In addition, by comparison, the treatment group began to appear the life ability improvement in the first at the end of the treatment, the control group in the second treatment began to appear obvious curative effect, the treatment group in improving the ability of patients with quick and significant.4. In remission in patients with limb pain, two treatments showed significant analgesic effect, wherein, the control group the total effective rate was92.11%,16cases were cured (42.11%), effective in19cases (50%), invalid3cases (7.89%), deterioration in0cases; the total effective rate in treatment group was97.43%, markedly in18cases (46.15%), effective in20cases (51.28%), invalid1cases (2.57%), deterioration in0cases. In contrast, patients in the treatment group relative significant pain relief, electro acupuncture combined with Acupoint Application in relieving physical pain is quick, and the effect is obvious.5. In the long term observation of curative effects of5,1months after the end of treatment, evaluation of upper limb motor function, Fugl-Meyer ADL integral and MPQ integral of the patients in the two groups, compared with the end of the treatment, there was no significant change (P>0.05), a relatively stable condition, no repeatedly, namely two kinds of therapy has clinical effect of long term.6. Quality of life scale data statistics:comparison of two groups of patients survival quality scale data was statistically significant (P<0.05), the treatment group and the control group compared with general form, improve health and vitality, the treatment group than the control group (P<0.05), but there was no significant difference in other aspects (P>0.05). Therefore, the quality of life in patients of the two groups after treatment were significantly improved, the treatment group than the control group.ConclusionBoth the galvano-acupuncture and acupoint application of Chinese medicine have definite curative effect on post-stroke shoulder-hand syndrome, by the analysis of the Fugl-Meyer motor function of upper limb scoring method, the index of MPQ, the index of ADL, and the index of SF-36. And the galvano-acupuncture plus the acupoint application of Chinese medicine can inforce the effect, with low relapse rate. So we should extend the application of the conbined treatment. |
