| Objective: To evaluate the clinical efficiency and safety of scalp points in treatment of Bell palsy, through a clinical studies of scalp electro-acupuncture, combining with body acupuncture treatment of severe acute phase of Bell palsy (pathogenic wind attacking exterior), thus providing an effective acupuncture treatment conducive to the promotion.Methods: 1. All selected 70 patients were randomly divided into scalp electro-acupuncture combined needle group (treatment group) and body acupuncture group (the control group), 35 cases each.Treatment Measures: Treatment group: body acupuncture: points on the affected side: Yangbai, Sibai, Zanzhu, Sizhukong, Quanliao, Xiaguan, Yingxiang, Dicang, Jiache, Jiachengjiang, Yifeng, Fengchi, and bilateral points: Hegu and Taichong; scalp points located on the first bilateral frontal, temporal, former slash under two fifths. Acupuncture and moxibustion: Within a week of disease onset, facial points are stabbed superficially 0.1-0.2 inch, with light stimulation, mild reinforcing and reducing manipulation, and purging method on Hegu and Taichong points with vertical insertion up to 1.0 inch. One week after onset: Yangbai, Zanzhu, Sizhukong, Yingxiang, Jiachengjiang horizontal needling, 0.5-1 inch, Sibai, Quanliao, Xiaguan , Yifeng, Fengchi, Hegu, Taichong, 1-1.5 inches vertical needling, penetration needling from Dicang to Jiache, with mild reinforcing and reducing method. Scalp acupuncture: after conventional disinfection fast needle insertion at about 30 degrees angle, with rapid piercing of the skin. When the needle reaches the tendinous part of epicranius muscle, the feeling of obstruction weakens, after which the needle is inserted deeper parallel to the skin up to the needed level, then applied quick twisting 200 times per min., lasting for about one minute, after which connecting HANS nerve stimulator. Connection Method: The polar electrodes of electro-acupuncture instrument were connected with the frontal, temporal, former slash needle handle on each side respectively. Treatment parameters: porosity wave frequency 2/15Hz, the intensity of stimulation should be light, enough for the patients to tolerate. Time of electro stimulation is 30 minutes. Control group: same acupuncture points as for the treatment group, without Scalp Acupuncture. Needling manipulation and time are same as for the treatment group.Observation index: 1. Main symptoms and signs of change of facial paralysis recommended by the House Brackmann (H-B) evaluation of facial nerve function classification system on Fifth International Facial Nerve Surgery seminar. 2. Blood routine test: taken one time before treatment and once after treatment.Effect Evaluation Standards: House Brackmann (H-B) evaluation of facial nerve function classification system recommended by Fifth International Light Facial Nerve Surgery seminar.Results: 1. Treatment of the two groups after each course of treatment compared with those before treatment, differences were statistically significant (P <0.01), the treatment group, three courses: total efficiency of 100%. Control group, the total efficiency 94.29%; 2. The treatment group compared with the control group: the first course of treatment had no significant difference (P>0.05). In the second and third treatment difference between the two groups was statistically significant (P <0.01); 3. Two groups in different stages of treatment, gender and efficiency tests were not significant (P>0.05); 4. Two groups in different stages of treatment, age and the effect of correlation analysis showed that in the treatment group: recuperation was related to age, correlation test with statistical significance. The younger the better resume, the greater the age the worse the recovery. Control group in the first and second courses, the recovery of the disease was age-related, correlation test with statistical significance, have also shown better recovery results among the younger patients. In the third treatment during the recovery stage of the disease: treatment with age-related testing without statistical significance; 5. Treatment group, different causes, distinct efficiency low in the following order: Bacterial infections (100%) better than Virus infection (75.0%) better than the unknown (50.0%). Control group, the different causes, distinct efficiency low in the following order: Bacterial infections (100%) better than Virus infection (54.5%) better than the unknown (42.8%). 6. The treatment of complication had no significant difference for the two groups (P>0.05).Conclusions: 1. Scalp electro acupuncture combining with body acupuncture treating severe paralysis Bell has good efficiency, its effect is better than body acupuncture treatment alone; 2. Scalp electro acupuncture combining with body acupuncture has obvious effects on severe Bell's palsy induced by bacterial infection; 3. Patients with severe Bell facial paralysis with different age have different recovery of facial nerve function; 4. Scalp electro acupuncture combining with body acupuncture treating severe paralysis Bell is safe, relatively easy and convenient for applying, thus beneficial for clinical generalization. |
