Yiqihuoxue Phlegm And Collaterals To Slow Down The Progress Of Chronic Obstructive Pulmonary Disease Clinical Research

Theoretical clinical and experimental studies on the treatment of chronicobstructive pulmonary disease (COPD) by traditional Chinese medicine(TCM) were made systematically in this paper. In theoretical study, this paper systematically induced and arranged theopinions about the literature record of collateral disease theory (CDT) sincethe period of the Internal Classis, and also summarized its contemporaryresearch progress. In addition, this paper introduced in detail the concept,risk factors, etiological factors, pathologic transformation, manifestation,prevention and cure of COPD. Based on the (GOLD),and combined with its up-to-dateresearch progress. A survey of COPD was made in detail in terms of itsconcept, risk factors, etiology and pathology, manifestation, prevention andtreatment program. Furthermore it also summarized the research progress ofprevention and cure of COPD, with TCM in recent 20 years. Finally, thispaper analyzed the correlation between the CDT and pulmonary diseases,and expounded Prof. Wu Weiping's scholastic thought of prevention andtreatment of COPD by means of nourishing qi, activating blood, dispersingphlegm and promoting the collaterals. In clinical study, Such items as Interleukin-1(IL-1),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),hyaluronic acid (HA),laminin (LN),procollagen type Ⅲpeptide(PIIIP),transforming factor-β(TGF-β) andb-fibroblast growth factor(b-FGF) in the sera of 105 patients with COPDwere examined and compared with that of 14 healthy people, and also thecorrelative analysis was made among these 8 items in sera. The resultsshowed that all the items except TNF-αin the sera of patients with COPDwere remarkably increased than that of healthy people. The level of HA was4 益气活血化痰通络法阻止慢性阻塞性肺疾病进展的临床研究significantly correlative with that of LN, PIIIP and TGF-β. 72 patients with COPD at stationary phase (level 0~II) were divided intotherapeutic group (35 cases) and control group (37 cases), randomly andsingle-blindedly. The control group was treated according to GOLDprogram, and the therapeutic group was treated by the compoundprescriptions of nourishing qi, activating blood, dispersing phlegm andpromoting the collaterals as well as by the standardized GOLD program.Clinical effect was evaluated by Quality of life, and all the indexes in thesera before and after treatment were compared. The results indicated that thecompound prescriptions used in the therapeutic group had the better effect indecreasing the levels of IL-8, HA, LN, PIIIP, TGF- β, b-FGF, andimproving the Quality of life of the COPD than the therapy used in thecontrol group. The therapeutic mechanism of the method of nourishing qi,activating blood, dispersing phlegm and promoting the collaterals wassupposed as that it intervened the airway remodeling and alleviated theprogress of airflow obstruction of COPD by suppressing the chemo taxis ofcytokines, regulating the proliferation of fibroblast cell and smooth musclecell and preventing the proliferation and plerosis of extracellular matrix.
